MSCT检查应用于结肠癌术前TNM分期诊断的影像学特点及效能分析

Imaging characteristics and efficacy analysis of MSCT examination in preoperative TNM staging diagnosis of colon cancer

目的:分析多层螺旋CT (MSCT)用于结肠癌术前TNM分期的影像学特点和诊断效能。方法:回顾性分析2022年1月—2023年5月单县海吉亚医院收治的经手术病理确诊的结肠癌患者80例的临床资料,患者均接受MSCT检查,总结其MSCT检查的影像学特点,并将病理结果当作参照,统计MSCT在术前T分期、N分期以及M分期中的诊断准确度。结果:80例患者均完成MSCT检查,获取的检查图像较为满意。MSCT检查期间病变类型多为溃疡型,占61.25%;60.00%患者存在肠壁增厚,40.00%患者存在肠腔狭窄,90.00%患者附近脂肪间隙模糊,12.50%患者存在中度强化,22.50%存在重度强化。将病理结果当作金标准,MSCT检查在T分期、N分期、M分期中的准确率分别是90.00%、85.00%、95.00%。结论:MSCT能提供出病变位置、病变类型、病变表现、附近脂肪间隙以及强化程度等情况,准确开展术前TNM分期,在临床上有着较高应用价值。

Objective To analyze the imaging characteristics and diagnostic efficacy of multi-slice spiral CT(MSCT)for preoperative TNM staging of colon cancer.Methods A retrospective analysis was conducted on the clinical data of a total of 80 colon cancer patients diagnosed by surgery and pathology admitted to Shanxian County Haijiya Hospital from January 2022 to May 2023,all of whom underwent MSCT examination.The imaging characteristics during the MSCT examination were summarized,and the pathological results were used as a reference to calculate the accuracy of MSCT in preoperative T staging,N staging,and M staging.Results All 80 patients completed MSCT examination and obtained satisfactory examination images.During MSCT examination,the type of lesion was mostly ulcerative,accounting for 61.25%;60.00%of patients had intestinal wall thickening,40.00%had intestinal stenosis,90.00%had blurred fat spaces near them,12.50%had moderate enhancement,and 22.50%had severe enhancement.Using pathological results as the gold standard,the accuracy of MSCT examination in T staging,N staging,M staging were 90.00%,85.00%,95.00%,respectively.Conclusion MSCT can provide information on the location,type,manifestation,adjacent fat spaces,and degree of enhancement of the lesion,accurately staging preoperative TNM,and has high clinical application value.
