Nesfatin-1、miR-155-5p表达与2型糖尿病肾病患者的相关性研究 被引量:1

摘要 目的探讨摄食抑制因子-1(Nesfatin-1)、微小核糖核酸-155-5p(miR-155-5p)与2型糖尿病肾病的相关性。方法选择2020年1月至2021年8月,本院收治的120例2型糖尿病肾病患者(观察组)、100例单纯2型糖尿病患者(无肾病对照组)和100例健康体检者(健康对照组)为研究对象。用酶联免疫吸附法、实时荧光定量聚合酶链反应(RT-PCR)分别对Nesfatin-1、miR-155-5p进行检测。比较3组的Nesfatin-1、miR-155-5p差异,探析2项指标与2型糖尿病肾病的关系。结果观察组Nesfatin-1、高密度脂蛋白胆固醇低于无肾病对照组、健康对照组,miR-155-5p、空腹血糖、糖化血红蛋白、胰岛素抵抗、三酰甘油、总胆固醇、低密度脂蛋白胆固醇、Cr高于无肾病对照组、健康对照组(P<0.05)。Nesfatin-1、miR-155-5p与空腹血糖、糖化血红蛋白、胰岛素抵抗、三酰甘油、总胆固醇、高密度脂蛋白胆固醇、低密度脂蛋白胆固醇、Cr有相关性(P<0.05)。Nesfatin-1、miR-155-5p联合诊断2型糖尿病肾病的曲线下面积AUC(0.841,95%CI 0.711~0.971)高于单一指标(P<0.05)。Nesfatin-1低表达、miR-155-5p高表达是糖尿病肾病的影响因素(P<0.05)。结论在2型糖尿病肾病中Nesfatin-1呈低表达,miR-155-5p呈高表达。Nesfatin-1、miR-155-5p与2型糖尿病肾病有关,若2型糖尿病患者的Nesfatin-1下降、miR-155-5p升高,反映受检者的肾病发生风险增加,肾功能可能出现损伤。 Objective To investigate the correlation between dietary inhibitory factor 1(Nesfatin-1)and micrornas 155-5p(miR-155-5p)and type 2 diabetic nephropathy.Methods From January,2020 to August,2021,120 patients with type 2 diabetic nephropathy treated in our hospital(observation group),100 patients with simple type 2 diabetes(control group without renal disease)and 100 healthy physical examination subjects(healthy control group)were selected as the study objects.Nesfatin-1 and miR-155-5p were detected in all subjects by enzyme-linked immunosorbent assay and real-time fluorescence quantitative polymerase chain reaction(RT-PCR),respectively.Levels of Nesfatin-1 and miR-155-5p among these three groups were compared to explore the relationship between the two indexes and type 2 diabetic nephropathy.Results Nesfatin-1 and high-density lipoprotein cholesterol in the observation group were lower than those in the control group without kidney disease and healthy control group,while miR-155-5p,fasting blood glucose,glycated hemoglobin,insulin resistance,triglyceride,total cholesterol,low-density lipoprotein cholesterol and Cr were higher than those in the control group without kidney disease and healthy control group(P<0.05).Nesfatin-1 and miR-155-5p were correlated with fasting blood glucose,glycated hemoglobin,insulin resistance,triacylglycerol,total cholesterol,high-density lipoprotein cholesterol,low-density lipoprotein cholesterol and Cr(P<0.05).AUC(0.841,95%CI 0.711-0.971)of combined Nesfatin-1 and miR-155-5p in the diagnosis of type 2 diabetic nephropathy was higher than that of single-index test(P<0.05).Low expression of Nesfatin-1 and high expression of miR-155-5p were the influencing factors of diabetic nephropathy(P<0.05).Conclusion In type 2 diabetic nephropathy,the expression of Nesfatin-1 is low while the expression of miR-155-5p is high.Nesfatin-1 and miR-155-5p are associated with type 2 diabetic nephropathy.When Nesfatin-1 is decreased and miR-155-5p is increased in type 2 diabetic patients,the risk of renal disease is increased and renal function may be impaired.
