牙周炎患者血清及龈沟液NLRP3 mRNA,IL-18,IL-1β的变化及临床意义

Changes of NLRP3 mRNA,IL-18,IL-1βin serum and gingival crevicular fluid of patients with periodontitis and its clinical significance

目的 探讨牙周炎患者NLRP3 mRNA、IL-18、IL-1β的变化及临床意义。方法 分别选取2020年6月至2021年6月收治的慢性牙周炎患者200例与健康者100例为观察组与对照组,两组均采集血清、龈沟液样本,检测NLRP3 mRNA表达量及IL-18、IL-1β水平。结果 观察组血清、龈沟液NLRP3 mRNA表达量、IL-18、IL-1β水平均高于对照组(P<0.05);血清、龈沟液NLRP3 mRNA表达量、IL-18、IL-1β水平与牙周指数均呈正相关(P<0.05);ROC分析显示,血清NLRP3 mRNA表达量(AUC=0.854,95%CI:0.792~0.915)、IL-18(AUC=0.745,95%CI:0.663~0.826)、IL-1β(AUC=0.791,95%CI:0.718~0.864)、龈沟液NLRP3 mRNA表达量(AUC=0.878,95%CI:0.822~0.935)、IL-18(AUC=0.818,95%CI:0.754~0.882)、IL-1β(AUC=0.811,95%CI:0.741~0.880)均是诊断牙周炎的重要因素(P<0.05)。结论 血清及龈沟液NLRP3 mRNA表达量及IL-18、IL-1β水平在牙周炎患者中明显升高,可作为诊断牙周炎的参考指标。

Objective To investigate the changes of NLRP3 mRNA,IL-18 and IL-1βof patients with periodontitis and its clinical significance.Methods A total of 200 patients with chronic periodontitis admitted from Jun 2020 to Jun 2021 and 100 healthy people were selected as observation group and control group,Serum and gingival crevicular fluid samples were collected from both groups and NLRP3 mRNA expression and IL-18,IL-1βlevels were detected.Results The NLRP3 mRNA expression level and IL-18,IL-1βin serum and gingival crevular fluid of observation group were higher than those of control group(P<0.05);There was a positive correlation between NLRP3 mRNA expression level,IL-18,IL-1βlevels and periodontal index(P<0.05).ROC curve analysis showed that NLRP3 mRNA expression level(AUC=0.854,95%CI:0.792~0.915),IL-18(AUC=0.745,95%CI:0.663~0.826),IL-1β(AUC=0.791,95%CI:0.718~0.864)in serum,NLRP3 mRNA expression level(AUC=0.878,95%CI:0.822~0.935),IL-18(AUC=0.818,95%CI:0.754~0.882),IL-1β(AUC=0.811,95%CI:0.741~0.880)in gingival crevular fluid were important factors in diagnosing periodontitis.Conclusion The levels of NLRP3 mRNA and IL-18,IL-1βin serum and gingival crevular fluid were significantly increased in patients with periodontitis.
