早期营养干预结合同理心干预对行结直肠癌根治术患者术后营养状态及免疫功能的影响

Effect of early nutrition intervention combined with empathy intervention on nutritional status and immune function of colorectal cancer patients after radical resection

目的探讨早期营养干预结合同理心干预对行结直肠癌根治术患者术后营养状态及免疫指标的影响。方法以2020年1月至2022年12月在广东省中医院珠海医院接受结直肠癌根治术后的168例患者作为研究对象,采用随机数字表法分为2组。对照组84例患者施以常规护理,观察组84例患者则在对照组干预方式的基础上施以早期营养干预结合同理心干预。比较2组的术后康复相关临床指标。结果观察组的术后排气时间和术后排便时间均早于对照组,住院时间短于对照组(P均<0.05)。术后1周和2周,2组的主观全面评价法评分、血清总蛋白水平、前白蛋白水平、免疫功能指标水平及中文版心理韧性调查量表评分均优于同组术后1 d,且观察组均优于同期对照组,均有统计学差异(P均<0.05)。观察组的术后并发症总发生率明显较对照组更低(P<0.05)。结论早期营养干预结合同理心干预可有效加快行结直肠癌根治术患者的术后康复进程,改善营养状态,提高机体免疫功能,增强心理韧性,并有助于降低术后并发症发生风险。

Objective To explore the effect of early nutrition intervention combined with empathy intervention on the nutritional status and immune indicators of patients after radical resection of colorectal cancer.Method A total of 168 patients who received radical resection of colorectal cancer at Guangdong Provincial Hospital of Chinese Medicine,Zhuhai from January 2020 to December 2022 were selected as the study subjects and divided into two groups using a random number table.The 84 patients in the control group carried out routine care,while the 84 patients in the observation group carried out early nutrition intervention combined with empathy intervention on the basis of the control group.The clinical indicators related to postoperative rehabilitation were compared between the two groups.Results The postoperative exhaust time and defecation time of the observation group were earlier than those of the control group,and the hospitalization time was shorter than that of the control group(all P<0.05).At 1 and 2 weeks after surgery,the Subjective Global Assessment score,serum total protein level,prealbumin level,immune indicator levels,and the Chinese version of the Connor-Davidson Resilience Scale score in the two groups were better than those at 1 day after surgery,and the observation group was superior to the control group,with statistical differences(all P<0.05).The total incidence of postoperative complications in the observation group was significantly lower than that in the control group(P<0.05).Conclusion Early nutrition intervention combined with empathy intervention can effectively accelerate the postoperative rehabilitation process,improve the nutritional status,enhance the immune function and psychological resilience,and help reduce the risk of postoperative complications in colorectal cancer patients after radical resection.
