中医定向透药治疗联合中药熏蒸在膝关节骨性关节炎患者中的应用

Application of directional penetration therapy combined with fumigation of traditional Chinese medicine on patients with knee osteoarthritis

目的分析中医定向透药治疗联合中药熏蒸在膝关节骨性关节炎(KOA)患者中的应用效果和价值。方法回顾性分析2020年1月至2022年11月扬州市中医院收治的共计98例KOA患者的病例资料,按护理方法的不同分入对照组(50例,给予常规护理)和观察组(48例,实施中医定向透药治疗联合中药熏蒸)。比较2组的临床疗效、疼痛程度、膝关节功能以及炎症因子水平。结果观察组的临床总有效率为97.92%,高于对照组的86.00%(P<0.05)。出院时和出院1个月后,2组的疼痛数字评分法(NRS)评分、膝关节肿胀和膝关节僵硬程度评分均较入院时降低,且观察组均较同期对照组更低,组内和组间差异均有统计学意义(P均<0.05)。出院时和出院1个月后,2组的白介素-6(IL-6)和肿瘤坏死因子水平-α(TNF-α)水平均较入院时降低,且观察组均较同期对照组更低,组内和组间差异均有统计学意义(P均<0.05)。结论中医定向透药治疗联合中药熏蒸对KOA患者的临床疗效提升、疼痛程度和膝关节功能改善以及炎症因子消退均可起到积极影响。

Objective To analyze the application effect and value of directional penetration therapy combined with fumigation of traditional Chinese medicine in patients with knee osteoarthritis(KOA).Methods The clinical data of 98 KOA patients admitted to Yangzhou Hospital of Traditional Chinese Medicine from January 2020 to November 2022 were retrospectively analyzed.According to the different nursing methods,they were divided into a control group(50 cases,given routine nursing)and an observation group(48 cases,given directional penetration therapy combined with fumigation of traditional Chinese medicine).The clinical efficacy,pain intensity,knee function,and inflammatory factor levels of the two groups were compared.Results The total clinical effective rate of the observation group was 97.92%,which was higher than 86.00%of the control group(P<0.05).At discharge and 1 month after discharge,the scores of Numerical Rating Scale(NRS),knee swelling and knee stiffness in both groups were lower than those at admission,and the observation group had lower scores than the control group.There were significant differences within and between groups(all P<0.05).At discharge and 1 month after discharge,the levels of interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)in both groups were lower than those at admission,and the observation group had lower scores than the control group.There were significant differences within and between groups(all P<0.05).Conclusion The combination of directional penetration therapy and fumigation of traditional Chinese medicine has a positive impact on improving the clinical efficacy,pain intensity and knee function,and decreasing the inflammatory factors in KOA patients.
