原发性高血压中医证型与血清Cys-C、Hcy相关性研究

Approach to correlativity between TCM syndrome types and serum Cys-C,Hcy of essential hypertension

目的:探讨原发性高血压中医证型与血清胱抑素C(Cys-C)、血同型半胱氨酸(Hcy)的相关性。方法:采用回顾性研究方法,纳入原发性高血压患者110例为研究对象,收集一般资料与临床资料,对患者进行中医证型分型,分析不同中医证型高血压病患者的病例数及构成比,以及不同中医证型之间血清Cys-C、Hcy的水平。结果:原发性高血压患者可辨证为肝阳上亢证、气血亏虚证、痰瘀互结证、肾精不足证4种证型,证型分布依次为痰瘀互结证﹥肾精亏虚证﹥气血亏虚证﹥肝阳上亢证。肾精亏虚证组血清Cys-C含量最高,气血亏虚证组含量最低;痰瘀互结证组血清Hcy含量最高,肝阳上亢证组含量最低。结论:原发性高血压病患者证型中痰瘀互结证占比最高,不同中医证型间的血清Cys-C、Hcy水平不同。

Objective:To approach the correlativity between TCM syndrome types and serum cystatin C(Cys-C),plasma homocysteine(Hcy)of essential hypertension.Methods:By the method of retrospective study,110 cases of included patients with essential hypertension were taken as researched objects,the general and clinical data were collected.TCM syndrome types of patients were classified according to TCM syndrome differentiation.The case number and constituent ratio of patients with hypertension of different TCM syndrome types were analyzed,and the differences of serum Cys-C and Hcy levels among different TCM syndrome types were analyzed.Results:The patients with essential hypertension could be divided into four types.syndrome of liver-yang hyperactivity,syndrome of deficiency of both qi and blood,syndrome of intermingled phlegm and blood stasis and syndrome of deficiency of kidney essence.In the order of syndrome type,the distribution of syndrome types was syndrome of intermingled phlegm and blood stasis﹥syndrome of deficiency of kidney essence﹥syndrome of deficiency of both qi and blood﹥syndrome of liver-yang hyperactivity.The content of serum Cys-C was the highest in the group of syndrome of deficiency of kidney essence,while the lowest in the group of syndrome of deficiency of both qi and blood.The content of serum Hcy was the highest in the group of syndrome of intermingled phlegm and blood stasis,while the lowest in the group of syndrome of liver-yang hyperactivity.Conclusion:The syndrome of intermingled phlegm and blood stasis accounts for the highest proportion in TCM syndrome types of patients with essential hypertension.The levels of serum Cys-C and Hcy are different among different TCM syndrome types.
