常规12导联心电图与24 h动态心电图检查诊断心律失常与检测起搏器功能的效果比较

Comparisons of the effects of 24 h dynamic electrocardiogram and conventional 12 lead electrocardiogram on the diagnosis of arrhythmia and the examination of pacemaker function

目的分析起搏器植入患者分别开展24 h动态心电图与常规12导联心电图诊断心律失常的效果差异。方法选取2017年5月至2022年5月广东省东莞市厚街医院的61例起搏器植入患者作为研究对象。全部患者在进入医院后均使用两种心电图检测方法,分别开展常规12导联心电图以及24 h动态心电图检查。比较两种检查方式的心律失常检出率以及不同心律失常类型、起搏器工作状态方面的检出率。结果24 h动态心电图心律失常检出率高于常规12导联心电图,差异有统计学意义(P<0.05)。24 h动态心电图在房性心律失常检出率为65.57%,室性心律失常检出率为62.30%,显示起搏器是否工作检出率为100.00%,起搏器工作模式多样性检出率为47.54%。常规12导联心电图在房性心律失常检出率为19.67%,室性心律失常检出率为8.20%,显示起搏器是否工作检出率为78.69%,起搏器工作模式多样性检出率为0.00%,24 h动态心电图在房性心律失常、室性心律失常、显示起搏器是否工作以及起搏器工作模式多样性方面检出率均高于常规12导联心电图,差异有统计学意义(P<0.05)。结论起搏器植入患者开展24 h动态心电图检查,能够有效检出心律失常,还可对不同类型心律失常进行检测,同时更能精确分析起搏器功能及工作模式多样性。

Objective To analyze the difference between 24 h dynamic electrocardiogram and conventional 12 lead electrocardiogram in diagnosing arrhythmia in patients with pacemaker implantation.Methods A total of 61 patients with pacemaker implantation in Houjie Hospital of Dongguan from May 2017 to May 2022 were selected as the study objects.After entering the hospital,all patients were treated with 12 lead electrocardiogram and 24 h dynamic electrocardiogram examination.The detection rate of arrhythmia,the detection rate of different arrhythmia types and pacemaker working status were compared between the two methods.Results The rate of arrhythmia in 24 h dynamic electrocardiogram was higher than that in conventional 12 lead electrocardiogram,and the difference was statistically significant(P<0.05).The detection rate of 24 h dynamic electrocardiogram in atrial arrhythmia was 65.57%,the detection rate of ventricular arrhythmia was 62.30%,the detection rate whether the pacemaker works was 100.00%,the detection rate of pacemaker working mode diversity was 47.54%.The detection rate of routine 12 lead electrocardiogram in atrial arrhythmia was 19.67%,the detection rate of ventricular arrhythmia was 8.20%,the detection rate of whether the pacemaker works was 78.69%,the detection rate of pacemaker working mode diversity was 0.00%.The detection rate of 24 h dynamic electrocardiogram in atrial arrhythmia,ventricular arrhythmia,whether the pacemaker works,and the diversity of pacemaker working mode were higher than that of conventional 12 lead electrocardiogram,and the differences were statistically significant(P<0.05).Conclusion 24 h dynamic electrocardiogram can detect arrhythmia effectively in patients with pacemaker implantation,and can also detect different types of arrhythmia,and can accurately analyze the diversity of pacemaker function and working mode.
