肺炎克雷伯菌肝脓肿合并脓毒性肺栓塞1例

A case of Klebsiella pneumoniae liver abscess combined with septic pulmonary embolism

脓毒性肺栓塞是指含有病原性微生物的栓子脱落后随血流进入肺动脉系统进而导致肺动脉的栓塞、肺梗死、局灶性肺部脓肿。脓毒性肺栓塞是一种发病较急、临床少见且严重的疾病,是肺栓塞比较罕见的类型,因其起病隐匿、早期症状不典型、缺乏明显的临床特异性,通常表现为呼吸系统的症状,如胸闷、憋气、呼吸急促等;加之临床医生对脓毒性肺栓塞缺乏相关了解,经验相对较少,临床诊疗工作中诊断困难,容易出现遗漏、忽视,而明确诊断时往往病情已经出现进展,临床上容易造成严重不良后果。威海市中医院近期收治1例肝脓肿合并脓毒性肺栓塞的患者,对该病例进行了临床总结及经验分析,旨在提高对该病的认知,改善疾病预后,详细病情资料及文献讨论如下。

Septic pulmonary embolism refers to the detachment of emboli containing pathogenic microorganisms that flow into the human pulmonary artery system with blood,leading to pulmonary embolism,pulmonary infarction,and focal pulmonary abscess.Septic pulmonary embolism is a rare and severe disease with acute onset,clinical rarity,and severity.It is a relatively rare type of pulmonary embolism.Due to its insidious onset,atypical early symptoms,and lack of obvious clinical septic pulmonary embolism specificity,it usually manifests as symptoms of the respiratory system,such as chest tightness,suffocation,and shortness of breath.In addition,clinical doctors lack relevant understanding of septic pulmonary embolism and have relatively little experience,making it difficult to diagnose in clinical diagnosis and treatment work,which is prone to omissions and neglect.When a clear diagnosis is made,the condition often progresses,which can easily lead to serious adverse consequences in clinical practice.The clinical experience of this case from Weihai Traditional Chinese Medicine Hospital was summarized and analyzed,aiming to improve understanding of the disease and improve itsprognosis.Detailed disease information and literature discussion are as follows.
