基于CiteSpace知识图谱的针刀治疗肩周炎可视化分析 被引量:2

摘要 目的:分析针刀疗法治疗肩周炎的研究进展与研究现状。方法:检索中国知网、万方数据库、维普数据库、中国生物医学文献数据库从2000年1月1日至2022年8月31日有关针刀治疗肩周炎的相关文献,应用NoteExpress对纳入文献进行统计归纳,完成数据整理,通过CiteSpace 5.7.R5分析该领域内的文献作者、研究机构、关键词的共现联系和聚类特征,绘制知识图谱。结果:共纳入文献1033篇,发文量大体呈增长趋势。发文量>3篇的机构有29所,主要研究机构有北京中医药大学、湖北中医药大学、南京中医药大学、中日友好医院、广西中医药大学、北京中医药大学东直门医院等机构。发文数量≥3的核心作者有25位,核心作者为郭长青、刘乃刚、张天民、张强、刘满震、刘福水、陈幼楠、李新洲、丁毅等。在针刀治疗肩周炎领域具有较高频数的关键词为“临床研究”“针刀松解”“手法治疗”等。25个关键词呈现突现,突现时间最近的5个关键词为“超微针刀”“运动功能”“疼痛评分”“超声引导”“研究进展”;突现的集中时间段为2018—2022年。针刀治疗肩周炎的研究热点主要集中在超声引导、疼痛评分、针刺方法、运动功能等方面。聚类分析显示近年来研究多沿袭以前的聚类研究,并未发展出新的较大聚类和明显的研究方向。时间线图表明大部分高频、高中介性的关键词集中出现在早期,而后期多在关键词基础上进行研究拓展。多个关键词聚类存在明显联系。针刀治疗肩周炎的相关研究可大致分为起始阶段、发展阶段、深化阶段3个阶段。起始阶段(2000—2007年)特点是节点多、连线密集、中心性强、研究主题较为丰富,且研究成果丰富;发展阶段(2010—2015年)特点是在既往研究的基础上,对针刀治疗肩周炎的研究更加深入,主题广泛扩展;深化阶段(2018—2022年)研究热点相对较少,但仍受到研究者的注意和关注。结论:2000—2022年针刀治疗肩周炎的研究取得了一定进展,研究热点主要集中在针刀不同治疗方式的临床应用方面,包括超微针刀、超声引导针刀等。研究内容偏向于临床研究,基础研究相对匮乏,且存在跨机构、跨团队合作交流较少等问题。超声引导下的针刀治疗是近年的研究热点,可能成为以后的重点研究方向。 Objective:To analyze the research progress and research status of needle-knife therapy for frozen shoulder.Methods:The relevant literature on needle-knife therapy for frozen shoulder from January 1,2000 to August 31,2022 was searched from China Knowledge Network,Wanfang,vip database,and China Biomedical Literature Database.NoteExpress was applied to statistically summarize the included literature and data.The literature was analyzed by CiteSpace 5.7.R5 for authors,research institutions,and the co-occurrence links and clustering features of keywords,and the knowledge map was drawn.Results:A total of 1033 articles were included,with a general trend of increasing number of articles.There were 29 institutions with a publication volume greater than 3 articles,and the main research institutions included Beijing University of Chinese Medicine,Hubei University of Chinese Medicine,Nanjing University of Chinese Medicine,China-Japan Friendship Hospital,Guangxi University of Chinese Medicine,and Dongzhimen Hospital of Beijing University of Chinese Medicine.There are 25 core authors with a number of publications≥3,and the core authors are GUO Changqing,LIU Naigang,ZHANG Tianmin,ZHANG Qiang,LIU Manzhen,LIU Fushui,CHEN Younan,LI Xinzhou and DING Yi.The keywords with high frequency were clinical research,needle-knife release and manipulative treatment.Totally 25 keywords showed emergence,and the 5 keywords with the most recent emergence time were ultramicrotomy,motor function,pain score,ultrasound guidance and research progress.The concentrated time period of emergence was 2018-2022.The hotspots of research on needle-knife for the treatment of frozen shoulder were mainly focused on ultrasound guidance,pain score,acupuncture methods and motor function.Cluster analysis showed that recent research has mostly followed previous clustering studies and has not developed new enough to form large clusters and obvious research directions.The timeline diagram shows that most of the high-frequency and highly mediated keywords were concentrated in the early years,while the later years mostly expanded based on the keyword.Multiple keyword clusters are clearly linked.Research related to needle-knife for the treatment of frozen shoulder can be roughly divided into 3 stages,the beginning stage,the development stage and the deepening stage.The starting stage(2000-2007)is characterized by many nodes,intensive linkage,strong centrality,relatively rich research themes,and abundant research results.The development stage(2010-2015)is characterized by more in-depth research on needle-knife treatment of frozen shoulder based on previous research,and extensive expansion of themes.The deepening stage(2018-2022)has relatively few research hotspots,but still receives attention and interest from researchers.Conclusion:Research on needle-knife in the treatment of frozen shoulder from 2000-2022 has made some progress,with research hotspots mainly focusing on the clinical application of different treatment modalities of needle-knife,including ultramicrotomy and ultrasound-guided needle-knife.The research content is biased towards clinical research,and there is a relative lack of basic research,but there are problems such as less cross-institutional and cross-team cooperation and communication.Ultrasound-guided needle-knife treatment is a hot research topic in recent years and may become a key research direction in the future.
