面向临床的便携式膝关节康复检测系统设计

摘要 目的 阐述面向临床的便携式膝关节康复检测系统的设计过程。方法 系统由支具、信号采集模块以及人机交互模块组成,根据设计要求确定柔顺性、阻力可变性、量程与精度等特性,设置系统变阻力测量模式以测量膝关节活动度(Range ofMotion,ROM)和肌力等关键功能数据,选取4名健康受试者参与系统性能测试实验和计算系统精度。结果 模拟测试现场结果表明,通常3个周期内动作获取稳定的ROM。系统性能测试结果表明,4名受试者膝关节ROM测量Rmae≤0.6°,Rme<1.5°;Tmae<0.15 Nm,Tme<0.4 Nm;3 d内受试者组内重测信度为0.8988~0.9511。系统ROM量程95°,肌力量程28.4Nm;系统所用弹簧刚度0.2989 Nm/°,系统包括三级阻力模式,对应量程分别是16.89、24.38、28.40 Nm。结论 本研究提出的测试方案具备有效的ROM测试效果,便携式膝关节康复检测系统可为患者提供安全、舒适的膝关节术后早期康复所需测量环境,有效降低下肢与支具间随机绑缚造成的活动度测量误差,满足膝关节术后康复的检测要求,符合临床对康复进展跟踪和康复方案评估的需求。 Objective To expound the development process of a portable knee joint rehabilitation detection system for clinical application.Methods The system was composed of brace,signal acquisition module and human-computer interaction module.According to the design requirements,the characteristics of flexibility,resistance variability,range and accuracy were determined,and the system variable resistance measurement mode was set to measure key functional data such as range of motion(ROM)and muscle strength of the knee joint.Four healthy subjects were selected to participate in the system performance test experiment and calculate the system accuracy.Results The simulation test results showed that typically within 3 cycles,ROM was obtained.The system performance test results indicated that the Rmae measured by ROM of knee joint for 4 subjects was≤0.6°,and the Rme<1.5°.Tmae<0.15 Nm,and the Tme<0.4 Nm.The retest reliability within the subject group within 3 d was between 0.8988 and 0.9511.The measuring range of ROM was 95°and the measuring range of muscle strength was 28.4 Nm.The spring stiffness of the system was 0.2989 Nm/°,and the system included three resistance modes with corresponding ranges of 16.89,24.38 and 28.40 Nm respectively.Conclusion The proposed test plan in this study has an effective ROM test effect.The portable knee joint rehabilitation assessment system can provide a safe and comfortable measurement environment for early postoperative knee joint rehabilitation of patients.It effectively reduces the measurement errors caused by random constraints between the lower limbs and brace,meeting the requirements for postoperative knee joint rehabilitation assessment.It also meets the clinical needs for tracking rehabilitation progress and evaluating rehabilitation programs.
