基于数据挖掘探讨谢春光治疗2型糖尿病用药规律

Study on the Medication Law of Xie Chunguang in the Treatment of Type 2 Diabetes Mellitus Based onData Mining

目的 基于数据挖掘方法分析谢春光教授治疗2型糖尿病的组方规律。方法 收集2022年3-9月谢春光教授于成都中医药大学附属医院名医堂及内分泌科门诊诊治的2型糖尿病患者病案,经筛选后建立数据库,采用古今医案云平台对处方药物使用频次、功效、性味归经进行统计,并进行聚类分析、关联规则及复杂网络分析,挖掘谢春光教授治疗2型糖尿病用药规律。结果 共纳入处方122首,涉及134味中药,其中高频药物有黄芪、山药、山萸肉、麦冬等,药性以寒、温为主,药味以甘、苦、辛居多,归经主要为肺经、肾经、肝经和胃经。药物功效以清热泻火、养阴生津、清热凉血、清热燥湿及益气补中为主。支持度较高的关联规则为山萸肉-山药、山药-炙黄芪和山萸肉-炙黄芪。聚类分析得到4组核心药物,复杂网络分析得到9味核心中药。结论 谢春光教授治疗2型糖尿病以“气阴两虚”为核心病机,标本同治,寒热并用,兼顾先后二天,从疾病源头遏制疾病进展,对临床治疗2型糖尿病有一定指导作用。

Objective To analyze the medication law of Professor Xie Chunguang in the treatment of type 2 diabetes mellitus.Methods Medical records of type 2 diabetes mellitus patients treated by Professor Xie Chunguang at the Mingyitang and Endocrinology Department of Chengdu University of Traditional Chinese Medicine Affiliated Hospital from March 2022 to September 2022 were collected.After screening,a database was established.The prescription data were analyzed using the Cloud Platform of Ancient and Modern Medical Cases to investigate the frequency of medication,the properties,tastes and meridians,drug clustering,drug associations,and complex network analysis.The aim was to uncover the medication law used by Professor Xie Chunguang in the treatment of type 2 diabetes mellitus.Results A total of 122 prescriptions were included,involving 134 types of Chinese materia medica.The high-frequency drugs included Astragali Radix,Dioscoreae Rhizoma,Corni Fructus,and Ophiopogonis Radix.The predominant properties of the drugs were cold and warm,while the tastes were mainly sweet,bitter,and pungent.The drugs were mainly categorized under the lung,kidney,liver,and stomach meridians.The therapeutic effects of the drugs were primarily focused on clearing heat and reducing fire,nourishing yin and promoting fluid production,clearing heat and cooling blood,clearing heat and drying dampness,and replenishing qi and invigorating the middle.The high-support association rules were Corni Fructus-Dioscoreae Rhizoma,Dioscoreae Rhizoma-Astragali Radix Praeparata cum Melle,and Corni Fructus-Astragali Radix Praeparata cum Melle.Clustering analysis identified four groups of core drugs,and complex network analysis revealed nine kinds of core Chinese materia medica.Conclusion Professor Xie Chunguang’s treatment of type 2 diabetes mellitus takes“deficiency of both qi and yin”as the core pathogenesis,treats both symptoms and root causes,uses both cold and heat,and takes into account of both congenital kidney and acquired spleen and stomach to curb disease progression from the source of the disease,which has a certain guiding role in the clinical treatment of type 2 diabetes mellitus.
