不孕症女性高危型HPV感染的危险因素及对T淋巴细胞亚群水平的影响

Risk factors for high-risk HPV infection in infertile women and influence on T lymphocyte subsets

目的探讨不孕症女性高危型人乳头瘤病毒(HPV)感染的危险因素及对T淋巴细胞亚群水平的影响,为临床提供诊治依据.方法选取2019年7月-2021年10月海口市第四人民医院收治的92例高危型HPV阳性不孕症女性为感染组,另选取104例HPV阴性不孕症女性为未感染组,分析感染高危型HPV的型别分布和危险因素及对T淋巴细胞亚群水平的影响.结果感染的高危型HPV型别中HPV16、HPV52及HPV58的占比较高,分别为41.30%、20.65%及18.48%;初次性生活年龄<20岁、吸烟、有宫颈糜烂史是不孕症女性高危型HPV感染的独立危险因素;感染组外周血CD3+、CD_(4)^(+)、辅助性T淋巴细胞1(Th1)水平均低于未感染组,CD_(8)^(+)、Th2水平均高于未感染组(P<0.05).结论不孕症女性高危型HPV感染以HPV16、HPV52及HPV58为主,初次性生活年龄<20岁、吸烟、有宫颈糜烂史是不孕症女性高危型HPV感染的独立危险因素,高危型HPV感染会导致患者T淋巴细胞亚群失衡.

OBJECTIVE To explore the risk factors for high-risk human papillomavirus(HPV)infection in the infertile women and observe the influence on T lymphocyte subsets so as to provide bases for clinical diagnosis and treatment.METHODS A total of 92 infertile women who were tested positive for high-risk HPV and treated in the Fourth People's Hospital of Haikou from Jul 2019 to Oct 2021 were assigned as the infection group,and 104 infertile women who were tested negative for HPV were chosen as the no infection group.The subtypes of high-risk HPV,risk factors for the infection and influence of the lymphocyte subsets were observed.RESULTS Among the interfile women infected with high-risk HPV,41.30%were infected with HPV16,20.65%infected with HPV52,18.48%infected with HPV58.The age of initial sexual life less than 20 years old,smoking and history of cervical erosion were the independent risk factors for the high-risk HPV infection in the infertile women.The levels of peripheral blood CD_(4)^(+),CD_(8)^(+)and helper T cell 1(Th1)of the infection group were lower than those of the no infection group,while the levels of CD。+and Th2 of the infection group were higher than those of the no infection group(P<0.05).CONCLUSION HPV16,HPV52 and HPV58 are the predominant subtypes of highrisk HPV among the infertile women.The age of initial sexual life less than 20 years old,smoking and history of cervical erosion are the independent risk factors for the high-risk HPV infection.The high-risk HPV infection may result in the imbalance of T lymphocyte subsets.
