摘要
目的探究乳腺癌改良根治术后感染患者外周血干扰素基因刺激蛋白(STING)、TANK结合激酶1(TBK1)、信号传导及转录激活蛋白6(STAT6)表达及意义.方法选取2020年2月-2021年2月湛江中心人民医院收治的乳腺癌患者90例,根据术后是否出现切口感染分为感染组27例,非感染组63例.分析感染病原菌分布特点,使用聚合酶链式反应(PCR)法检测术前外周血STING、TBK1、STAT6 mRNA相对表达量,比较两组术后5 d炎症因子降钙素原(PCT)、C-反应蛋白(CRP)、白细胞介素6(IL-6)水平,绘制受试者工作特征(ROC)曲线分析STING、TBK1及STAT6对术后感染的预测价值.结果27例术后切口感染患者共分离病原菌33株,其中革兰阳性菌21株,占63.64%,革兰阴性菌12株,占36.36%;感染组外周血STING、TBK1、STAT6 mRNA相对表达量及CRP、PCT、IL-6水平均高于非感染组(P<0.05);ROC曲线显示,外周血STING、TBK1及STAT6表达预测术后切口感染的曲线下面积(AUC)分别为0.933、0.909、0.894.结论乳腺癌改良根治术后切口感染患者外周血STING、TBK1及STAT6呈异常表达,且对预测感染有较高的效能.
OBJECTIVE To explore the expressions of stimulator of interferon genes(STING),TANK-binding kinase 1(TBK1)and signal transducer and activator of transcription 6(STAT6)in peripheral blood of modified radical mastectomy patients with postoperative infection.METHODS Totally 90 breast cancer patients who were treated in the Central People's Hospital of Zhanjiang from Feb 2020 to Feb 2021 were enrolled in the study and were divided into the infection group with 27 cases and the non-infection group with 63 cases according to the occurrence of postoperative incision infection.The distribution of pathogens was analyzed,the relative expression levels of STING,TBK1 and STAT6 mRNA in peripheral blood were detected by polymerase chain reaction(PCR)before the surgery.The levels of inflammatory factors procalcitonin(PCT),C-reactive protein(PCT)and interleukin-6(IL-6)were compared between the two groups after the surgery for 5 days.The values of STING,TBKl and STAT6 in prediction of postoperative infection were analyzed by receiver operating characteristic(ROC)curve.RESULTS Totally 33 strains of pathogens were isolated from 27 patients with postoperative incision infection,21(63.64%)of which were gram-positive bacteria,and 12(36.36%)were gram-negative bacteria The relative expression levels of peripheral blood STING,TBK1,STAT6 mRNA and the levels of CRP,PCT and IL-6 were higher in the infection group than in the non-infection group(P<0.05).ROC curve analysis showed that the areas under curves(AUCs)of the peripheral blood STING,TBK1 and STAT6 were respectively 0.933,0.909 and 0.894 in prediction of the postoperative incision infection.CONCLUSION The modified radical mastectomy patients with postoperative incision infection show abnormal expressions of peripheral blood STING,TBK1 and STAT6,which have high efficiencies in prediction of the infection.
