多形态眶隔脂肪重置术治疗黑眼圈伴泪沟畸形临床观察

Clinical Observation of Multiform Orbital Fat in the Resetting Treatment of Dark Circles with Lacrimal Groove Deformity

目的 观察多形态眶隔脂肪重置术治疗混合型黑眼圈伴泪沟畸形的临床疗效及安全性。方法 选取2021年7月-2022年5月我院收治的27例混合型黑眼圈伴泪沟畸形女性患者,均经结膜松解泪槽韧带,将眶脂多形态、多层次重置于黑眼圈和泪沟区域,观察患者满意度、疗效及术后并发症情况。结果 27例患者均随访术后6个月,患者总满意率92.59%(25/27),25例患者可见黑眼圈消退或范围变小,色泽变白,泪沟平整。2例患者术后出现结膜充血,冰敷后消退,其余患者术后未有持续肿胀、黄染,感染、脂肪栓塞等并发症发生。6个月后下睑黑眼圈区L^(*)值为(57.96±5.76)较术前(47.56±4.62)明显提高,差异有统计学意义(P<0.05)。a^(*)b^(*)值对比差异低无统计学意义(P>0.05)。TTRS评分与术前比下降(P<0.05),提示泪沟畸形矫正。结论 多形态眶隔脂肪重置术对下睑混合型黑眼圈伴轻中度泪沟畸形、皮肤松弛不重的年轻患者安全、有效。

Objective To observe the clinical effect of multiform orbital fat in the resetting treatment of mixed dark circles with lacrimal groove deformity.Methods 27 female patients with mixed dark circles with lacrimal groove deformity treated in our hospital from July 2021 to May 2022 were selected.All of them were treated with conjunctival ligaments of the lacrimal groove,and the orbital ffifat was used to replace the dark circles and lacrimal groove deformity in multiple forms and layers.The patient satisfaction,efficacy and postoperative complications were observed.Results All the 27 patients were followed up 6 months after operation,the total satisfaction rate of the patients was 92.59%(25/27).In 25 patients,the dark circles disappeared or the range became smaller,the color became light,and the lacrimal groove was smooth.Conjunctival congestion occurred in two patients after surgery,which was removed by ice treatment,while the other patients did not have continuous yellow staining,swelling,infection,fat embolism and other complications after surgery.The L^(*) value of lower eyelid dark circle area was(57.96±5.76)after 6 months compared with(47.56±4.62)before fffisurgery,and the difference was statistically significant(P fiff<0.05).There was no significant difference in a^(*)b^(*) values(P>0.05).The TTRS score was lower than that before surgery(P<0.05),suggesting correction of lacrimal groove deformity.Conclusion Multiform orbital fffat replacement is safe and effective for young patients with mixed dark circles with mild-moderate lacrimal groove deformity and less severe skin sag.
