舒芬太尼、丙泊酚复合右美托咪定在假体隆乳术中麻醉维持的效果

Effect of sufentanil,propofol combined with dexmedetomidine on anesthesia maintenance during breast augmentation surgery with implants

目的 探讨舒芬太尼、丙泊酚复合右美托咪定用于假体隆乳术中麻醉维持的效果。方法 回顾性收集2019年1月-2022年12月在我院行假体隆乳术的564例患者的临床资料,根据麻醉方式不同分为A组(舒芬太尼、丙泊酚进行麻醉,271例)和B组(舒芬太尼、丙泊酚复合右美托咪定麻醉,293例)。比较两组的麻醉效果[麻醉苏醒时间、苏醒后Ramsay评分、术后VAS评分]、术中血流动力学指标[心率(HR)、平均动脉压(MAP)、呼吸频率(RR)、脉搏血氧饱和度(SpO_(2))]及不良反应。结果B组麻醉苏醒时间较A组短,苏醒后Ramsay评分、术后2hVAS评分、术后6h VAS评分、术后12h VAS评分较A组低(P<0.05);B组T1、T2时HR、MAP、RR、SpO_(2)相比T0时差异不明显(P>0.05),A组T1、T2时HR、MAP、RR、SpO_(2)相比T0时均较低(P<0.05)。B组不良反应总发生率较A组低(P<0.05)。结论 舒芬太尼、丙泊酚复合右美托咪定应用于假体隆乳术中麻醉效果较好,有助于稳定术中血流动力学变化,且术后不良反应较少,具有较好的安全性。

Objective To explore the effffect of sufentanil,propofol,and dexmedetomidine in maintaining anesthesia during breast augmentation surgery with prostheses.Methods Clinical data 564 patients who underwent breast augmentation surgery with prostheses in our hospital from Jan 2019 to Dec 2022 were retrospectively collected.All the patients were divided into group A(anesthesia with sufentanil and propofol,271 cases)and group B(anesthesia with sufentanil and propofol combined with dexmedetomidine,293 cases)based on anesthesia methods.The anesthesia effects[anesthesia awakening time,Ramsay score after awakening,VAS score after surgery],intraoperative hemodynamic indicators[heart rate(HR),mean arterial pressure(MAP),respiratory rate(RR),pulse oxygen saturation(SpO_(2))]and adverse reactions were compared between the two groups.Results The anesthesia recovery time in Group B was shorter than that in Group A,and the Ramsay score,VAS score of postoperative 2h/6h/12h were lower than those in Group A(P<0.05).There were no difffferences in HR,MAP,RR,and SpO_(2) between T1 and T2 in Group B compared to T0(P>0.05),while the HR,MAP,RR,and SpO_(2) were all lower in Group A compared to T0(P<0.05).The total incidence of adverse reactions in Group B was lower than that in Group A(P<0.05).Conclusion The combination of sufentanil,propofol,and dexmedetomidine has a good anesthesia effffect during breast augmentation surgery with implants,which can help to stabilize intraoperative hemodynamic changes,and has fewer postoperative adverse reactions,indicating good safety.
