错颌畸形中前牙覆颌与下颌角、下颌平面角及腭平面倾斜度相关性研究

A study on the correlation between anterior teeth overdenture and mandibular angle, mandibular plane angle, and palatal plane inclination in malocclusion

目的 探讨错颌畸形中前牙覆颌与下颌角、下颌平面角及腭平面倾斜度相关性,为临床口腔美学修复、效果与预后评估提供依据。方法 选取2019年1月-2022年12月我院口腔科诊治的63例错颌畸形患者作为研究组,并以同期到院体检的50名牙周健康志愿者为对照组,均接受头颅侧位片检查,对比分析前牙覆颌、下颌角、下颌平面角(SN-MP)、腭平面倾斜度(PP-FH)的差异,并通过Pearson分析相关性。结果 通过测定,研究组的前牙覆颌、下颌角、SN-MP、FH-MP、PP-FH分别是(3.22±0.21)mm、(117.34±2.23)°、(34.30±2.44)°、(26.40±2.52)°、(11.76±0.20)°,对照组分别为(2.49±0.21)mm、(120.01±1.04)°、(39.42±2.17)°、(36.11±1.31)°、(3.20±0.13)°,组间差异存在统计学意义(P <0.05)。通过Pearson相关性分析,前牙覆颌与下颌角、SN-MP、FH-MP均呈负相关(r分别为-0.733、-0.403、-0.512,P <0.05),与PP-FH不具相关性(r为0.063,P> 0.05)。结论 错颌畸形患者的前牙覆颌随着下颌角变大而减小,随着下颌平面角增大而减小,但与腭平面倾斜度不相关,有助于临床口腔正畸治疗方案制定和美学效果评估。

Objective To explore the correlation between anterior teeth overdenture and mandibular angle,mandibular plane angle,and palatal plane inclination in malocclusion,and to provide a basis for clinical oral aesthetic restoration,effffectiveness,and prognosis evaluation.Method 63 patients with malocclusion diagnosed and treated in our hospital's dental department from January 2019 to December 2022 were selected as the study group,and 50 periodontal health volunteers who underwent physical examination at the same time were selected as the control group.All patients underwent lateral head X-ray examination,and the differences in anterior teeth overlap,mandibular angle,mandibular plane angle(SN-MP),and palatal plane inclination(PP-FH)were compared and analyzed through Pearson analysis.Result Through measurement,the anterior teeth overlap,mandibular angle,SN-MP,FHMP,and PP-FH of the study group were(3.22±0.21)mm,(117.34±2.23)°,(34.30±2.44)°,(26.40±2.52)°,and(11.76±0.20)°,respectively.The control group was(2.49±0.21)mm,(120.01±1.04)°,(39.42±2.17)°,(36.11±1.31)°,and(3.20±0.13)°,with statistically signifificant difffferences between the groups(P<0.05).Through Pearson correlation analysis,there was a negative correlation between anterior overbite and mandibular angle,SN-MP,FH-MP(r=-0.733,-0.403,-0.512,P<0.05),but no correlation with PP-FH(r=0.063,P>0.05).Conclusion The anterior teeth overlap of patients with malocclusion decreases as the mandibular angle increases and decreases as the mandibular plane angle increases,but it is not related to the inclination of the palatal plane,which is helpful for the development of clinical orthodontic treatment plans and aesthetic evaluation.
