肺炎支原体肺炎患儿免疫球蛋白及T淋巴细胞水平变化的临床意义

Clinical Significance of Changes in Immunoglobulin and T Lymphocyte Levels in Children with Mycoplasma Pneumoniae Pneumonia

摘要 目的 分析肺炎支原体肺炎(mycoplasma pneumoniae pneumonia,MPP)患儿的免疫球蛋白、T淋巴细胞水平改变特点,并分析其临床意义。方法 回顾性分析2022年4月—2023年4月徐州市矿务集团总医院收治的60例MPP患儿的临床资料,以患儿喘息、发热、咳嗽、咳痰等症状不同分为观察1组(症状明显组,25例)、观察2组(潜伏期或恢复期组,35例),选取60例健康儿童为对照组。比较3组免疫球蛋白、T淋巴细胞亚群水平及肺功能。结果 观察1组免疫蛋白G、免疫蛋白A、免疫蛋白M分别为(9.34±1.38)g/L、(1.06±0.17)g/L、(1.65±0.24)g/L均高于观察2组及对照组,3组对比差异有统计学意义(F=16.305、16.332、24.900,P<0.05)。3组CD4+、CD4+/CD8+、CD3+水平,第1秒用力呼气容积、第1秒用力呼气容积/用力肺活量比较,差异有统计学意义(P<0.05)。结论 MPP患儿病情不同可发生不同程度的免疫功能异常、T淋巴细胞活化及肺功能损害,积极监测患儿免疫球蛋白表达水平、T淋巴细胞活化比例有助于评估病变程度。 Objective To analyze the changes in immunoglobulin and T lymphocyte levels in children with mycoplasma pneumoniae pneumonia(MPP),and analyze their clinical significance.Methods A retrospective analysis was con⁃ducted on the clinical data of 60 children with MPP admitted to Xuzhou Mining Group General Hospital from April 2022 to April 2023.Different symptoms such as wheezing,fever,cough,and phlegm were divided into observation group 1(25 cases in the symptomatic group)and observation group 2(35 cases in the latent or recovery period group),with 60 healthy children selected as the control group.Compared the levels of immunoglobulins,T lymphocyte sub⁃sets,and lung function among three groups.Results The levels of IgG,IgA,and IgM in observation group 1 were(9.34±1.38)g/L,(1.06±0.17)g/L,and(1.65±0.24)g/L,respectively,higher than those in observation group 2 and the control group,the difference was statistically significant(F=16.305,16.332,24.900,P<0.05).The levels of CD4+,CD4+/CD8+,and CD3+among the three groups compared to FEV1 and FEV1/FVC,the differences was statistically significant(P<0.05).Conclusion Different degrees of immune dysfunction,T lymphocyte activation and lung function damage may occur in children with MPP in different conditions.Actively monitoring the expression level of immunoglobulin and the proportion of T lymphocyte activation in children is helpful to evaluate the degree of disease and prognosis.
