不同病情严重性癫痫患者与血清B细胞活化因子、血清脑源性神经营养因子、胶质纤维酸性蛋白水平的相关性

Correlation between Levels of Serum B-cell Activating Factor, Serum Brain-derived Neurotrophic Factor and Glial Fibrillary Acidic Protein in Patients with Epilepsy of Different Severity

摘要 目的 研究不同病情严重性癫痫患者与血清B细胞活化因子(B cell activation factor, BAFF)、血清脑源性神经营养因子神经营养因子(brain derived neurotrophic factor, BDNF)、胶质纤维酸性蛋白(glial fibrillary acidic protein, GFAP)水平的相关性。方法 选择惠州市中心人民医院于2020年12月—2022年12月收治的癫痫患者74例作为癫痫组。按照国立癫痫严重程度评分量表(National Epilepsy Severity Scale, NHS3)分级标准,将74例癫痫患者分成全身强直阵挛发作组(30例)和部分性发作组(44例),以45例同期体检健康者为对照组。分别测定其BDNF、BAFF、GFAP的水平,分析癫痫患者BDNF、BAFF、GFAP水平与病情严重性的相关性。结果 癫痫组BDNF水平(6.35±2.26)ng/mL低于对照组,而BAFF水平(9.05±2.14)ng/mL和GFAP水平(3.43±0.57)ng/L均高于对照组,差异有统计学意义(t=7.485、15.174、15.094,P<0.05);全身强直阵挛发作组中BDNF的水平与部分性发作组比较,明显降低,而BAFF水平、GFAP水平较部分性发作组明显升高,差异有统计学意义(P<0.05);BDNF与癫痫患者病情严重程度成负相关(r=-0.250,P=0.032);BAFF、GFAP与癫痫患者病情严重程度成正相关(r=0.395、0.584,P<0.05)。结论 癫痫患者的血清BDNF、BAFF和GFAP的检测结果均与正常人群的检测结果的有差异,并与患者的疾病严重性有明显的相关性。 Objective To study the relationship between serum B cell activation factor(BAFF),serum brain derived neurotrophic factor(BDNF),glial fibrillary acidic protein(GFAP)in patients with epilepsy of different severity.Methods A total of seventy-four patients with epilepsy admitted to Huizhou Central People's Hospital from Decem⁃ber 2020 to December 2022 were selected as epileptic group.According to the national epilepsy severity scale(NHS3)grading criteria,74 patients with epilepsy were divided into general tonic-clonic seizure group(30 cases)and partial seizure group(44 cases),and 45 healthy subjects who underwent physical examination during the same period were selected as control group.The levels of BDNF,BAFF and GFAP were measured respectively,and the correlation between the levels of BDNF,BAFF and GFAP and the severity of epilepsy was analyzed.Results The level of BDNF (6.35±2.26) ng/mL in epileptic group was lower than that in control group, while the level of BAFF (9.05± 2.14) ng/mL and GFAP (3.43±0.57) ng/L in epileptic group were higher than that in control group, and the difference was statistically significant (t=7.485, 15.174, 15.094, P<0.05). Compared with the partial seizure group, the level of BDNF in the generalized tonic-clonic seizure group was significantly decreased, the level of BAFF and GFAP were significantly increased compared with the partial seizure group, and the difference was statistically significant (P< 0.05). BDNF was negatively correlated with the severity of epilepsy (r=-0.250, P=0.032). BAFF and GFAP were posi⁃ tively correlated with the severity of epilepsy (r=0.395, 0.584, P<0.05). Conclusion The detection results of serum BDNF, BAFF and GFAP in epilepsy patients were different from those in normal population, and were significantly correlated with the severity of the disease.
