分析地佐辛联合丙泊酚在腹腔镜胆囊切除术患者麻醉中的应用

Application of Dezocine Combined with Propofol in Anesthesia of Patients Undergoing Laparoscopic Cholecystectomy

目的探讨腹腔镜胆囊切除术中应用地佐辛联合丙泊酚麻醉的应用效果和价值。方法选取2021年1月—2023年1月齐齐哈尔医学院附属第三医院行腹腔镜下胆囊切除术治疗的62例患者为研究对象,全部患者均需气管插管全身麻醉,以信封法要求随机、平均保持均衡性分组,参考组(31例,丙泊酚麻醉维持)、研究组(31例,地佐辛+丙泊酚麻醉维持)。比较两组各时间下生命体征指标、疼痛、苏醒期躁动、镇静评分及临床指标。结果研究组苏醒时血压水平、心率水平低于参考组,差异有统计学意义(P<0.05)。两组各时间段的血氧饱和度比较,差异无统计学意义(P>0.05)。研究组拔管时、拔管后5 min、拔管后30 min疼痛评分分别为(2.10±0.42)分、(2.31±0.54)分、(2.70±0.75)分,均低于参考组(4.05±1.03)分、(3.75±0.98)分、(3.34±0.61)分,差异有统计学意义(t=9.761、7.165、3.659,P<0.05)。研究组拔管时、拔管后5 min、拔管后30 min疼痛评分、苏醒期躁动评分低于参考组,差异有统计学意义(P<0.05)。两组拔管时、拔管后5 min、拔管后30 min时镇静评分对比,差异无统计学意义(P>0.05)。研究组丙泊酚用量少于参考组,苏醒时间、定向力恢复时间短于参考组,差异有统计学意义(P<0.05);两组患者拔管时间对比,差异无统计学意义(P>0.05)。结论地佐辛联合丙泊酚麻醉在腹腔镜胆囊切除术患者中应用效果较好,镇静效果突出,能够维持患者术中生命体征稳定,术后恢复快、镇痛效果好。

Objective To investigate the application effect and value of dezocine combined with propofol anesthesia in laparoscopic cholecystectomy.Methods From January 2021 to January 2023,62 patients who underwent laparoscopic cholecystectomy in the Third Affiliated Hospital of Qiqihar Medical College were selected as the research objects.All patients needed general anesthesia with tracheal intubation.They were randomly and evenly divided into reference group(31 cases,propofol anesthesia maintenance)and study group(31 cases,dezocine+propofol anesthesia mainte⁃nance)according to the envelope method.The vital signs,pain,emergence agitation,sedation score and clinical indica⁃tors were compared between the two groups at each time.Results The blood pressure level and heart rate level of the study group were lower than those of the reference group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in blood oxygen saturation between the two groups at each time period(P>0.05).The pain scores of the study group at extubation,5 min after extubation,and 30 min after extubation were(2.10±0.42)points,(2.31±0.54)points,and(2.70±0.75)points,respectively,which were lower than the reference group(4.05±1.03)points,(3.75±0.98)points,(3.34±0.61)points,the difference was statistically significant(t=9.761,7.165,3.659,P<0.05).The pain scores and agitation scores during recovery at extubation,5 min after extubation,30 min after extubation in the study group were lower than those in the reference group,and the differences were statisti⁃cally significant(P<0.05).There was no statistically significant difference in sedation score between the two groups at extubation,5 min after extubation and 30 min after extubation(P>0.05).The dosage of propofol in the study group was less than that in the reference group,and the recovery time and directional force recovery time were shorter than those in the reference group,the differences were statistically significant(P<0.05).There was no statistically significant dif⁃ference in extubation time between the two groups(P>0.05).Conclusion Dezocine combined with propofol anesthesia in laparoscopic cholecystectomy patients has a good application effect,outstanding sedation effect,can maintain the stability of patients'vital signs during the operation,rapid recovery after surgery,and good analgesic effect.
