微型钢板与克氏针内固定治疗掌指骨骨折的疗效分析

Treatment of Metacarpophalangeal Fracture with Microplate and Kirsch Nail Internal Fixation

目的对比研究掌指骨骨折患者采用微型钢板内固定与克氏针内固定两种方式实施手术治疗的临床效果。方法选择2021年11月—2022年11月在江苏省无锡市宜兴市第五人民医院接受手术治疗的60例掌指骨骨折患者为研究对象,根据手术所选择的内固定方式的不同将其分成对照组和治疗组。对照组30例患者采用克氏针内固定技术实施手术治疗;治疗组30例患者采用微型钢板内固定技术实施手术治疗。对比两组治疗效果相关评价指标。结果治疗组患者手术出血量为(125.29±7.40)mL,少于对照组,差异有统计学意义(t=19.143,P<0.05)。治疗组操作时间(57.48±12.64)min、术后疼痛消失时间(5.19±0.86)d、手部功能复常时间(53.38±2.96)d、住院总时间(7.48±1.64)d均短于对照组,差异有统计学意义(t=6.948、12.339、16.253、11.213,P<0.05)。治疗组治疗总有效率高于对照组,差异有统计学意义(P<0.05)。治疗组术后不同时间点炎症反应相关指标、指主动屈曲度、疼痛程度评分的改善幅度大于对照组,差异有统计学意义(P<0.05)。治疗组术后不良反应发生率低于对照组,差异有统计学意义(P<0.05)。结论微型钢板内固定方式治疗掌指骨骨折临床效果显著,值得关注。

Objective To compare the clinical effects of microplate internal fixation and Kirsch nail internal fixation on metacarpophalangeal fractures.Methods 60 patients with metacarpophalangeal fracture who received surgical treat⁃ment in Wuxi Yixing Fifth People's Hospital from November 2021 to November 2022 were selected as the research ob⁃jects and divided into control group and treatment group according to the different internal fixation methods chosen.In the control group,30 patients were treated with Kirsch nail internal fixation technique,and in the treatment group,30 patients were treated with microplate internal fixation technique.The relevant evaluation indexes of treatment effect were compared between the two groups.Results The amount of surgical bleeding in the treatment group was(125.29±7.40)mL,less than that in the control group,and the difference was statistically significant(t=19.143,P<0.05).The operation time of the treatment group was(57.48±12.64)min,the time for pain to disappear after surgery was(5.19±0.86)d,the time for hand function to return to normal was(53.38±2.96)d,and the total hospital stay was(7.48±1.64)d,which were shorter than those of the control group,and the difference was statistically significant(t=6.948,12.339,16.253,11.213,P<0.05).The total effective rate of the treatment group was higher than that of the control group,and the difference was statistically significant(P<0.05).The improvement of inflammatory response indexes,active flexion and pain degree scores in the treatment group was greater than that in the control group at different time points,and the difference was statistically significant(P<0.05).The incidence of postoperative adverse reactions in treatment group was lower than that in control group,and the difference was statistically significant(P<0.05).Conclusion Micro⁃plate internal fixation in the treatment of metacarpophalangeal fractures has remarkable clinical effect,which is worthy of attention.
