分析半导体激光辅助牙周基础治疗对中重度慢性牙周炎的治疗效果

Effect of Semiconductor Laser Assisted Periodontal Primary Therapy on Moderate to Severe Chronic Periodontitis

目的评估半导体激光+牙周基础治疗应用在中重度慢性牙周炎(chronic periodontitis,CP)患者中的效果。方法选取2022年1月—2023年3月泰安市口腔医院收治的90例中重度CP患者,参照随机数表法划分对照组(45例,行牙周基础治疗)、观察组(45例,加用半导体激光治疗),评价组间牙周参数、龈沟液炎性因子、牙龈疼痛情况[视觉模拟评分法(Visual Analog Scoring,VAS)]、龈下菌群。结果治疗后,观察组牙周参数[龈沟出血指数(sulcus bleeding index,SBI)、临床附着丧失(clinical attachment loss,CAL)、牙周探诊深度(probing depth,PD)]、龈沟液炎性因子[白细胞介素-1(interleukin-1,IL-1β)、肿瘤坏死因子-α(tumor necrosis factor-alpha,TNF-α)、白细胞介素-6(interleukin-6,IL-6)]、VAS评分低于对照组,差异有统计学意义(P<0.05)。观察组乳杆菌(5.21±0.45)CFU/mL、梭杆菌(5.22±0.78)CFU/mL、拟杆菌(4.99±0.56)CFU/mL数量少于对照组的(5.87±0.56)CFU/mL、(6.71±0.89)CFU/mL、(5.70±0.62)CFU/mL,差异有统计学意义(t=6.163、8.446、5.701,P<0.05)。结论对中重度CP患者实施半导体激光+牙周基础治疗,可以控制龈下菌群繁殖,降低龈沟液中炎性因子表达,改善牙周状况,减轻疼痛感。

Objective To evaluate the effect of semiconductor laser plus periodontal primary therapy in patients with moderate to severe chronic periodontitis(CP).Methods A total of ninety patients with moderate to severe CP admitted to Tai'an Stomatological Hospital from January 2022 to March 2023 were selected and divided into control group(45 cases,receiving periodontal basic treatment)and observation group(45 cases,add semiconductor laser treatment)according to random number table method.Periodontal parameters,gingival crevicular fluid inflammatory factors,gum pain conditions[Visual Analog Scoring(VAS)]and subgingival microflora were evaluated.Results After treatment,the observation group of periodontal parameters[sulcus bleeding index(SBI),clinical attachment loss(CAL),probing depth(PD)],crevicular fluid inflammatory factors[interleukin-1(IL-1β),tumor necrosis factor-α(TNF-α),interleukin6(IL-6)]and VAS were lower than those of the control group,and the difference was statistically significant(P<0.05).The lactobacillus(5.21±0.45)CFU/mL,clostridium(5.22±0.78)CFU/mL and bacteroides(4.99±0.56)CFU/mL were lower than those of control group(5.87±0.56)CFU/mL,(6.71±0.89)CFU/mL and(5.70±0.62)CFU/mL,and the difference was statistically significant(t=6.163,8.446,5.701,P<0.05).Conclusion Semiconductor laser plus periodontal basic treatment for moderate and severe CP patients can control the proliferation of subgingival flora,reduce the expression of inflammatory factors in gingival crevicular fluid,improve periodontal status and alleviate pain.
