膝关节镜下探查清理术对退行性膝骨性关节炎患者的治疗效果及不良反应发生率探究

Investigation of the Therapeutic Effect of Knee Arthroscopic Exploratory Cleaning in Patients with Degenerative Knee Osteoarthritis and the Incidence of Adverse Effects

目的分析膝关节镜下探查清理术对退行性膝骨性关节炎患者治疗效果及不良反应发生率产生的影响。方法方便选取2019年1月—2022年4月福建省厦门市第五医院收治的108例退行性膝骨性关节炎患者作为研究对象,用随机数表法分为两组,每组54例。对照组患者给予膝关节腔内注射玻璃酸钠注射液治疗,观察组患者给予膝关节镜下探查清理术联合玻璃酸钠注射液治疗。比较两组患者术前、术后膝关节疼痛情况、各项指标水平及不良反应发生率。结果术后1、6个月观察组患者视觉模拟评分(Visual Analogue Scales,VAS)低于对照组,术后膝关节评分(Hospital for Special Surgery,HSS)高于对照组,差异有统计学意义(P<0.05)。术后1年两组患者VAS评分对比,差异无统计学意义(P>0.05)。术后观察组患者膝关节骨性关节炎分级及关节力线内翻、外翻水平均低于对照组,差异有统计学意义(P<0.05)。观察组患者不良反应发生率(1.85%)低于对照组(14.81%),差异有统计学意义(χ^(2)=4.364,P=0.037)。结论膝关节镜下探查清理术在退行性膝骨性关节炎患者治疗中具有较高的临床应用价值,可有效改善膝关节功能和疼痛程度,且安全性较高。

Objective To analyze the impact of knee arthroscopic exploratory cleaning on the outcome and incidence of adverse effects in patients with degenerative knee osteoarthritis.Methods 108 patients with degenerative knee osteoarthritis admitted to the Fifth Hospital of Xiamen City,Fujian Province from January 2019 to April 2022 were conveniently selected as the study subjects.They were divided into two groups using a random number table method,with 54 patients in each group.The control group patients were treated with injection of sodium hyaluronate injection into the knee joint cavity,while the observation group patients were treated with knee arthroscopic exploration and cleaning surgery combined with sodium hyaluronate injection.Compared the preoperative and postoperative knee joint pain,levels of various indicators,and incidence of adverse reactions between two groups of patients.Results At 1 and 6 months,the visual analog scale(VAS)of the observation group patients was lower than that of the control group,and after surgery the hospital for special surgery(HSS)was higher than that of the control group,the difference was statistically significant(P<0.05).There was no statistically significant difference in VAS scores between the two groups of patients one year after surgery(P>0.05).The postoperative grade of knee osteoarthritis,as well as the average joint force line varus and valgus in the observation group were lower than those in the control group,the difference was statistically significant(P<0.05).The incidence of adverse reactions in the observation group(1.85%)was lower than that in the control group(14.81%),the difference was statistically significant(χ^(2)=4.364,P=0.037).Conclusion The knee arthroscopic exploration and cleaning surgery has high clinical application value in the treatment of patients with degenerative knee osteoarthritis,which can effectively improve knee joint function and pain level,and has high safety.
