自我感受负担对肺癌免疫治疗患者生活质量的影响

Influence of self-perceived burden on the quality of life in patients with lung cancer immunotherapy

目的 调查肺癌免疫治疗患者生活质量现状,并探究自我感受负担对生活质量的影响,以及心理痛苦和焦虑所发挥的中介效应。方法 选取2022年6-12月该院肺癌免疫治疗患者278例,采用一般资料调查表、自我感受负担量表(SPBS)、心理痛苦温度计(DT)、医院用焦虑量表(HADS)及肺癌患者生活质量测定量表(FACT-L)进行分析,并借助软件检验心理痛苦和焦虑在自我感受负担与生活质量间的并列中介作用。结果 SPBS、DT、HADS评分依次为(29.69±5.90)、(3.72±2.11)、(7.81±4.33)分。FACT-L评分为(74.85±15.80)分,其中生理状况、功能状况、社会/家庭状况维度评分依次为(12.36±3.77)、(13.77±4.23)、(18.76±4.77)分。SPBS评分、DT评分、HADS评分和FACT-L评分之间呈显著相关(P<0.05)。心理痛苦和焦虑在自我感受负担与生活质量中分别起部分中介作用,间接效应值分别为-0.09、-0.21,并列中介效应占总效应的57.69%。结论 肺癌免疫治疗患者自我感受负担通过心理痛苦和焦虑的并列中介作用对生活质量产生影响。

Objective To investigate the current status of quality of life in patients with lung cancer immunotherapy,and to explore the impact of self-perceived burden on quality of life,as well as the mediating effect of psychological distress and anxiety.Methods A total of 278 patients with lung cancer immunotherapy in this hospital from June to December 2022 were selected and analyzed by general information questionnaire,Self-Perceived Burden Scale(SPBS),Distress Thermometer(DT),Hospital Anxiety and Depression Scale(HADS),and Functional Assessment of Cancer Therapy-Lung cancer(FACT-L).The collateral mediating effects of psychological distress and anxiety between self-perceived burden and quality of life was examined by software.Results The SPBS,DT,and HADS scores were(29.69±5.90),(3.72±2.11),and(7.81±4.33)respectively.The FACT-L score was(74.85±15.80),among which the scores of physiological status,functional status,and social/family status were(12.36±3.77),(13.77±4.23),and(18.76±4.77),respectively.There was significant correlation between SPBS score,DT score,HADS score,and FACT-L score(P<0.05).Psychological distress and anxiety played a partial mediating role in self-perceived burden and quality of life,respectively,with indirect effect values were-0.09 and-0.21,respectively.The collateral mediating effect accounted for 57.69%of the total effect.Conclusion The self-perceived burden of patients with lung cancer immunotherapy affects the quality of life through the collateral mediating effect of psychological distress and anxiety.
