宫腔镜下子宫内膜息肉切除术对患者孕激素水平、子宫内膜容受及炎症反应的影响

Effect of Hysteroscopic Endometrial Polypectomy on Patients'Progesterone Levels,Endometrial Tolerance,and Inflammatory Response

目的分析宫腔镜下子宫内膜息肉切除术对子宫内膜息肉患者炎症反应、孕激素水平、子宫内膜容受产生的影响。方法方便选取2022年1—12月黑龙江省医院妇科收治的124例子宫内膜息肉患者,将其按照抽签法分为两组,对照组62例予以传统宫腔镜下刮宫术,观察组62例予以宫腔镜下子宫内膜息肉切除术。对两组子宫内膜容受指标、炎症反应指标、孕激素水平进行比较。结果术后,观察组搏动指数、阻力指数均低于对照组,差异有统计学意义(P<0.05);术后,观察组C-反应蛋白、肿瘤坏死因子-α、白细胞介素-6均低于对照组,差异有统计学意义(P<0.05);术后,观察组FSH、E_(2)均高于对照组,差异有统计学意义(P<0.05);术后,观察组子宫内膜厚度、月经量和月经间隔恢复正常时间均优于对照组,差异有统计学意义(P<0.05)。观察组并发症发生率(4.84%)低于对照组(17.74%),差异有统计学意义(χ^(2)=5.153,P<0.05)。结论宫腔镜下子宫内膜息肉切除术有利于改善子宫内膜息肉患者的炎症反应、子宫内膜受容、孕激素水平,预后效果良好,安全性高。

Objective To analyze the effects of hysteroscopic resection of endometrial polyps on inflammatory response,progesterone level,and endometrial tolerance produced in patients with endometrial polyps.Methods A total of 124 patients with endometrial polyps admitted to the gynecology department of Heilongjiang Provincia Hospital from January to December 2022 were conveniently selected and divided into two groups according to the lottery method.62 patients in the control group received traditional hysteroscopic curettage,and 62 patients in the observation group received hysteroscopic endometrial polyp resection.Endometrial receptivity,inflammatory response and progesterone levels were compared between the two groups.Results After surgery,the pulse index and resistance index of the observation group were lower than those of the control group,and the difference was statistically significant(P<0.05).After surgery,C-reactive protein,tumor necrosis factor-αand interleukin-6 in the observation group were lower than those in control group,and the difference was statistically significant(P<0.05).After surgery,FSH and E_(2) in observation group were higher than those in control group,and the difference was statistically significant(P<0.05).After surgery,Endometrial thickness,menstrual volume and menstrual interval in the observation group were better than those in the control group,and the difference was statistically significant(P<0.05).The complication rate of observation group(4.84%)was lower than that of control group(17.74%),and the difference was statistically significant(χ^(2)=5.153,P<0.05).Conclusion Hysteroscopic resection of endometrial polyps is conducive to improving the inflammatory response,endometrial capacitation and progesterone level of patients with endometrial polyps,with a good prognosis and high safety.
