高危型HPV检测及TCT检查在宫颈癌筛查中的应用效果分析

Analysis of Application Effect of High-risk HPV Detection and TCT Examination in Cervical Cancer Screening

目的 探讨高危型人乳头瘤病毒(human papilloma virus,HPV)检测及液基薄层细胞学检查(thinprep cytology test,TCT)在宫颈癌筛查中联合应用的效果。方法 选择2019年1月-2021年12月在本院接受宫颈癌筛查的26324例女性受检者作为研究对象,对其进行HPV初筛,再对HPV初筛阳性病例进行阴道镜检查或TCT,统计宫颈癌筛查结果。结果 26324例女性经HPV初筛,共检出2547例HPV阳性,包括HPV16/18型阳性409例、其他类型HPV阳性2138例,占筛查总人数的9.67%。那么,只需要这部分人进行后续的TCT检查。HPV16/18型阳性409例中,204例患者经阴道镜检查确诊6例宫颈癌、110例宫颈癌前病变、88例阴性。其他类型HPV阳性2138例中,有1589例患者接受TCT,TCT结果显示1164例阴性、425例阳性,425例TCT阳性患者经阴道镜检查确诊2例宫颈癌、92例宫颈癌前病变、200例阴性。结论 高危型HPV检测联合TCT检查对宫颈癌及癌前病变具有良好的筛查价值,在临床上值得推广。

Objective To explore the application effect of high-risk Human papilloma virus(HPV)detection and Thinprep cytology test(TCT)in cervical cancer screening.Methods 26324 cases of female subjects who underwent cervical cancer screening in our hospital from January 2019 to December 2021 were selected for HPV primary screening,and then colposcopy or TCT were performed for HPV primary screening positive cases,and the results of cervical cancer screening were counted.Results A total of 2547 HPV positive cases(including 409 cases of HPV16/18 and 2138 cases of other types of HPV)were detected in 26324 women after HPV screening.Among 409 cases of HPV16/18 positive,204 cases were confirmed by colposcopy,6 cases of cervical cancer,110 cases of cervical precancerous lesions and 88 cases were negative.Among 2138 cases of other types of HPV positive,1589 patients received TCT,the results of TCT showed that 1164 cases were negative and 425 cases were positive.425 cases of TCT positive patients were confirmed by colposcopy,2 cases of cervical cancer,92 cases of cervical precancerous lesions and 200 cases were negative.Conclusion High risk HPV detection combined with TCT has good screening value for cervical cancer and precancerous lesions.
