超声引导下臂丛神经阻滞复合右美托咪定在骨科手术中的应用研究

Application Research of Ultrasound-guided Brachial Plexus Block Combined with Dexmedetomidine in Orthopedic Surgery

目的 探究超声引导下臂丛神经阻滞复合右美托咪定在骨科手术中的应用价值。方法 以骨科手术患者80例作为对象,采用抽签法随机分为对照组、研究组,每组各40例。对照组实施超声引导下臂丛神经阻滞麻醉,研究组应用超声引导下臂丛神经阻滞复合右美托咪定麻醉。对比两组患者的心率与血压指标、麻醉优良率、手术指标、术后恢复指标、疼痛程度、认知功能。结果 比较心率、血压指标,研究组在手术结束时、气管插管拔管不同阶段的各项指标均优于对照组(P<0.05);研究组麻醉优良率、并发症发生率均优于对照组(P<0.05)。研究组术中出血量与术中输血量均较对照组少,差异有统计学意义(P<0.05)。研究组的自主恢复呼吸时间、苏醒时间短于对照组,差异有统计学意义(P<0.05)。研究组术后1h、术后3h的疼痛评分明显低于对照组,差异有统计学意义(P<0.05)。比较麻醉后12h、麻醉后24h的认知功能评分,研究组更高(P<0.05)。结论 骨科手术患者实施超声引导下臂丛神经阻滞复合右美托咪定麻醉,具有显著效果。

Objective To explore application value of ultrasound guided brachial ple xus block combined with dexmedetomidine in orthopedic surgery.Methods The paper chose 80 patients with orthopedic surgery as study group,control group was treated with ultrasound guided brachial plexus block anesthesia,while study group with ultrasound guid ed brachial plexus block combined with dexmedetomidine anesthesia.Heart rate and blood pressure indicators,anesthesia excellence rate,surgical indicators,postoperative recovery indicators,pain level and cognitive function werer compared between two groups.Results Indexes of heart rate and blood pressure comparison between study group and control group showed(P<0.05).Study group had better excellent rate of anesthesia and higher incidence of complications than control group(P<0.05).Intraoperative bleeding and blood transfusion in study group was(794.82±102.83)and(476.91±19.89)mL,while that of control group was(1079.34±116.72)and(592.34±19.81)mL,with statistically significant difference(P<0.05).Spontaneous recovery breathing time and recovery time of research group was shorter than control group,with statistically significant difference(P<0.05).Pain scores of study group at 1 hour and 3 hours after surgery were significantly lower than control group,with statistically significant difference(P<0.05).Cognitive function scores at 12 hours and 24 hours after anesthesia in study group were higher(P<0.05).Conclusion Ultrasound guided brachial plexus block combined with dexmedetomidine anesthesia can achieve significant effect on orthopedic surgery patients.
