补肾活血汤联合雷火灸治疗股骨头缺血性坏死临床观察

Bushen Huoxue Decoction Combined with Thunder Fire Moxibustion in the Treatment of Ischemic Necrosis of Femoral Head

目的探析采用补肾活血汤加减联合雷火灸治疗股骨头缺血性坏死的效果。方法选择2019年3月—2021年3月在南昌市洪都中医院治疗的78例股骨头缺血性坏死患者作为研究对象,其中2019年3月—2020年3月的39例患者作为对照组,采用骨肽片治疗;2020年4月—2021年3月的39例患者作为治疗组,采用补肾活血汤加减联合雷火灸治疗。比较分析治疗后2组患者的髋关节疼痛情况[视觉模拟量表评分(VAS)]、髋关节活动度和外周血清细胞因子[肿瘤坏死因子-α(TNF-α)、血管内皮生长因子(VEGF)]变化情况,观察2组患者的治疗总有效率。结果治疗后,治疗组的VAS评分低于对照组,且髋关节活动度高于对照组(P<0.05);治疗组的TNF-α水平低于对照组,且VEGF水平高于对照组(P<0.05);治疗组治疗总有效率89.74%(35/39)高于对照组的71.79%(28/39)(P<0.05)。结论补肾活血汤加减联合雷火灸治疗股骨头缺血性坏死,可以缓解患者痛感,改善患者髋关节活动度,调整外周血清细胞因子状态,提高效果。

Objective To explore the effect of Bushen Huoxue decoction combined with thunder fire moxibustion on ischemic necrosis of femoral head.Methods A total of 78 patients with avascular necrosis of the femoral head treated in Nanchang Hongdu Hospital of Traditional Chinese Medicine from March 2019 to March 2021 were selected as the study subjects,39 patients from March 2019 to March 2020 were selected as the control group and were treated with bone peptide tablets.From April 2020 to March 2021,39 pa⁃tients were treated with Bushen Huoxue decoction combined with thunder fire moxibustion.The hip pain status[visual analogue scale(VAS)],hip motion and peripheral blood serum cytokines(tumor necrosis factor-α(TNF-α)and vascular endothelial growth factor(VEGF))of the two groups were compared and analyzed after treatment,and the total effective rate of the two groups was observed.Re⁃sults After treatment,VAS score in the treatment group was lower than that in the control group,and hip motion was higher than that in the control group(P<0.05).The level of TNF-αin treatment group was lower than that in control group,and the level of VEGF was higher than that in control group(P<0.05).The total effective rate of the treatment group was 89.74%(35/39),which was higher than that of the control group(71.79%,28/39)(P<0.05).Conclusion Bushen Huoxue decoction combined with thunder fire moxibustion can relieve the pain,improve the range of motion of hip joint,adjust the status of peripheral blood clearance factor and improve the therapeutic effect of ischemic necrosis of femoral head.
