老年冠心病患者PCI术后发生心绞痛的相关因素分析

Analysis of related factors of angina pectoris in elderly patients with coronary heart disease after PCI

目的:探讨老年冠心病患者经皮冠脉介入(PCI)术后发生心绞痛的相关因素。方法:选取2019年1月-2020年12月因冠心病收治入我院并接受PCI治疗的老年患者220例。按术后有无发生心绞痛分为心绞痛组(34例)和非心绞痛组(186例)。分析比较两组患者入院时基本人口学资料、心功能指标、冠脉造影与治疗指标。通过单因素与多因素logistics回归分析老年冠心病患者PCI术后并发心绞痛的独立危险因素。结果:年龄>75岁、吸烟、高血压、糖尿病、高脂血症、低水平LVEF、出院后停止二联抗血小板治疗、未采取大剂量他汀类药物为老年冠心病PCI术后发生心绞痛的独立危险因素。结论:老年冠心病患者PCI术后发生心绞痛的危险因素较多,年龄>75岁、吸烟、高血压、糖尿病、高脂血症、低LVEF、出院后停止二联抗血小板治疗以及未选用大剂量他汀类药物者更容易复发心绞痛。

Objective:To investigate the related factors of angina pectoris in elderly patients with coronary heart disease after percutaneous coronary intervention(PCI).Methods:220 elderly patients with coronary heart disease who were admitted to our hospital and received PCI from January 2019 to December 2020 were selected.Angina pectoris was divided into angina pectoris group(34 cases)and non-angina group(186 cases).The basic demographic data,cardiac function indexes,coronary angiography and treatment indexes were analyzed and compared between the two groups.The independent risk factors of angina pectoris after PCI in elderly patients with coronary heart disease were analyzed by univariate and multifactorial logistics regression.Results:Age>75 years old,smoking,hypertension,diabetes,hyperlipidemia,low level of LVEF,discontinuing dual antiplatelet therapy after discharge,and not taking high-dose statins were independent risk factors for angina pectoris after PCI.Conclusion:The risk factors for angina pectoris after PCI are higher in elderly patients with coronary heart disease.Patients with age>75 years old,smoking,hypertension,diabetes,hyperlipidemia,low LVEF,discontinuation of antiplatelet therapy after discharge and non-use of high-dose statins are more likely to relapse angina pectoris.
