可视化患者交接板的设计及在麻醉复苏患者管理中的应用

Design of a Visual Patient Handover Board and Its Application in the Management of Patients Recoveredfrom Anesthesia

目的 自行设计可视化患者交接板,并探讨其在手术患者交接及转运管理中的应用效果,以增强患者转运安全性。方法 选取2021年1月至2022年1月在海军军医大学第一附属医院接受手术治疗并在麻醉恢复室复苏的患者作为研究对象。以2021年1-6月收治的3 540例患者作为对照组,采用常规手术交接核查表交班;以2021年7月至2022年1月收治的4 097例患者作为观察组,采用常规手术交接核查表结合可视化患者交接板进行交班。比较两组患者在麻醉恢复室停留期间不良事件的发生率,并通过调查相关医护人员了解其对患者交接板的使用满意度。结果 使用可视化患者交接板后,患者在麻醉恢复室期间的不良事件发生率较使用前有所降低,转出延误率由26.35%下降至18.82%,去氧饱和率由1.13%下降至0.32%,管道滑脱事件发生率由0.45%下降至0.07%,差异均有统计学意义(P<0.05)。对22名麻醉医师、30名手术医师、44名手术室护士及6名麻醉恢复室护士的调查结果显示,其对可视化患者交接板的总体使用满意率为93.14%。结论 可视化患者交接板的使用,有利于增加护士对患者病情的掌握程度,并可引导其针对潜在不良事件进行预防及处理,可有效提升麻醉复苏护理质量。

Objective To design a visual patient handover board and explore its effect on the management of surgical patient handover and transfer,so as to enhance the safety of patient transfer.Methods The selected patients underwent surgical treatment in Changhai Hospital from January 2021 to January 2022 and they had their consciousness recovered in an anesthesia recovery room.A total of 3540 patients who were admitted to the hospital from January to June 2021 were divided into the control group,and a routine surgical handover checklist was used for their handover.A total of 4097 patients who were admitted to the hospital from July 2021 to January 2022 were divided into the observation group,and the routine surgical handover checklist+a visual patient handover board was used for their handover.The incidence of adverse events during the stay in the anesthesia recovery room was compared between the two groups,and the satisfaction rate of related health providers with the use of the patient handover board was surveyed.Results After the use of the visual patient handover board,the incidence of adverse events of patients in the anesthesia recovery room decreased compared with that before use,the delay rate of transfer-out decreased from 26.35%to 18.82%,the blood oxygen desaturation rate decreased from 1.13%to 0.32%,and the incidence of tube slippage decreased from 0.45%to 0.07%,with statistical significance(P<0.05).The survey of 22 anesthesiologists,30 surgeons,44 theatre nurses and 6 nurses in the anesthesia recovery room showed that the overall satisfaction rate with the use of the visual patient handover board was 93.14%.Conclusion The use of the visual patient handover board helps to increase nurses'understanding of patients'condition,guide them to prevent and deal with potential adverse events,and improve the nursing quality during anesthesia recovery.
