运用气机升降理论治疗高危肺结节探析

Analysis on the Treatment of High Risk Pulmonary Nodules with theTheory of Ascending and Descending Qi Movement

气机升降理论是指导临床诊治疾病的基本理论之一,其内容包括升降机制的研究、升降相关的核心脏腑等,这与高危肺结节的病因病机、结癌转化的病理过程及治疗等有着密切的联系。气机升降失调是引起肺结节发生、发展的基本病理特点,其中肺失肃降是首要病理环节,肝失疏泄、脾胃枢机不利是重要协同要素,三焦郁闭则为肺结节结癌转化的病态环境。因此,本文以调气解毒思想为指导,提出以补气、行气、降气为高危肺结节的基本治则,同时制定相应治法、方药,以便为高危肺结节的防治及早期肺癌中医药防控方案的制定提供借鉴思路。

The theory of ascending and descending qi movement is one of the basic theories of traditional Chinese medicine(TCM)guiding the clinical diagnosis and treatment of diseases.Its contents include the mechanism study of the theory,the core viscera related to ascending and descending of qi,etc.,and this is closely related to the etiology and pathogenesis,pathological process and treatment of high-risk pulmonary nodules.The imbalance of qi movement is the basic pathological feature that leads to the occurrence and development of pulmonary nodules,in which failure of lung in purification and down-sending is the primary pathological link,failure of liver in free coursing and disturbance of spleen and stomach in pivot mechanism are important synergistic factors,and triple energizers stagnation is the pathological environment for the transformation of lung nodules into cancer.Therefore,under the guidance of the idea of regulating qi and detoxication,the authors proposed that the basic principles for the treatment of high-risk pulmonary nodules should be to tonify qi,move qi and downbear qi,and in the meantime,formulation of corresponding treatment methods and formulae should be performed,so as to provide reference ideas for the prevention and treatment of high-risk pulmonary nodules and the formulation of early lung cancer prevention and control programs with TCM.
