导乐分娩联合体位管理在分娩中的应用效果

Application Effect of Doula Delivery Combined with Position Management in Delivery

目的:探讨导乐分娩联合体位管理在分娩中的应用效果。方法:选取2020年1—12月于东阿县人民医院进行自然分娩的产妇100例作为研究对象,采用随机数字表法分为对照组和观察组,各50例。对照组应用体位管理,观察组在对照组基础上实施导乐分娩管理。比较两组产妇焦虑情况、总产程时间、产后2 h出血量、剖宫产率以及两组新生儿窒息情况。结果:分娩前,两组焦虑评分比较,差异无统计学意义(P>0.05);分娩后,两组焦虑评分均低于分娩前,且观察组低于对照组,差异有统计学意义(P<0.05)。观察组总产程时间短于对照组,产后2 h出血量少于对照组,差异有统计学意义(P<0.05)。观察组剖宫产率低于对照组,差异有统计学意义(P=0.008)。两组新生儿Apgar评分≥8分、5~7分、<5分占比比较,差异无统计学意义(P>0.05)。结论:导乐分娩联合体位管理在分娩中的应用效果显著,能够缓解产妇焦虑情绪,缩短产程时间,降低出血量及剖宫产率,并且不会增加新生儿窒息情况。

Objective:To explore the application effect of doula delivery combined with position management in delivery.Methods:A total of 100 parturients who underwent spontaneous delivery in Dong'e County People's Hospital from January 2020 to December 2020 were selected as the study subjects.They were divided into control group and observation group according to random number table method,with 50 cases in each group.The control group implemented position management and the observation group implemented doula delivery management on the basis of control group.Maternal anxiety,total duration of labor,blood loss at 2 h postpartum,cesarean section rate,and neonatal asphyxia in the two groups were compared.Results:Before delivery,there was no significant difference in anxiety scores between the two groups(P>0.05).After delivery,the anxiety scores in two groups were lower than those before delivery,and the observation group was lower than the control group,and the difference was statistically significant(P<0.05).The total duration of labor in the observation group was shorter than that in the control group,and the blood loss at 2 h postpartum in the observation group was less than that in the control group,and the difference was statistically significant(P<0.05).The cesarean section rate in the observation group was lower than that in the control group,and the difference was statistically significant(P=0.008).There was no significant difference in the proportion of neonates with Apgar scores of≥8,5-7 and<5 points between the two groups(P>0.05).Conclusion:The application effect of doula delivery combined with position management in delivery is significant,which can relieve the anxiety of parturients,shorten the duration of labor,reduce the blood loss and cesarean section rate,and will not increase neonatal asphyxia.
