摘要
目的研究银-羟基磷灰石(Ag-HA)涂层种植体对牙周炎大鼠牙周炎症、种植体骨结合能力的影响。方法取40只雄性大鼠构建牙周炎模型,将造模成功牙周炎大鼠随机分为牙周炎组5只、Ag-HA涂层组18只和HA涂层组17只。牙周炎组大鼠不给予任何干预,Ag-HA涂层组植入Ag-HA涂层种植体,HA涂层组植入HA涂层种植体。比较术前及术后1周、2周、4周、8周各组大鼠龈沟出血指数、牙龈指数、探诊深度、炎症因子[血清肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-1β(IL-1β)]水平,检测各组大鼠术后1周、2周、4周、8周的骨结合能力[血清碱性磷酸酶(ALP)水平、种植体-骨结合率]。结果Ag-HA涂层组和HA涂层组术后1周、2周、4周、8周的龈沟出血指数、牙龈指数、探诊深度和术后4周、8周的血清TNF-α、IL-6、IL-1β水平均明显低于同组术前及同期牙周炎组(P均<0.05),且Ag-HA涂层组术后1周、2周、4周的探诊深度和术后4周、8周的血清TNF-α、IL-6、IL-1β水平均明显低于同期HA涂层组(P均<0.05);Ag-HA涂层组和HA涂层组术后1周、2周、4周、8周的血清ALP水平均明显高于同期牙周炎组(P均<0.05),Ag-HA涂层组和HA涂层组术后1周、2周的血清ALP水平比较差异均无统计学意义(P均>0.05),Ag-HA涂层组术后4周、8周的血清ALP水平和术后2周、4周、8周的种植体-骨结合率均明显高于同期HA涂层组(P均<0.05)。结论Ag-HA涂层种植体可减轻牙周炎症反应,提高种植体-骨结合能力。
Objective It is to study the effect of silver-hydroxyapatite(Ag-HA)coated implants on the inflammation,mineralization ability and osseointegration ability of rats with periodontitis.Methods Forty male rat were selected to establish models of periodontitis,and the successfully modeled rats with periodontitis were randomly divided into periodontitis group(n=5),Ag-HA coating group(n=18)and HA coating group(n=17).The rats in the periodontitis group were not treated with any therapy,the rats in the Ag-HA coating group were implanted with Ag-HA-coated titanium plate implants,and the rats in the HA group were implanted with HA-coated titanium plate implants.The gingival sulcus bleeding index,gingival index,probing depth,and the levels of inflammatory factors[serum tumor necrosis factor-α(TNF-α),interleukin-6(IL-6),interleukin-1β(IL-1β)]of the rats in each group were compared before surgery,and at 1,2,4,and 8 weeks after surgery,and the osseointegration capacity[serum alkaline phosphatase(ALP)levels,implant-bone incorporation rate]of the rats in every group was examined at 1,2,4,and 8 weeks after surgery.Results The gingival sulcus bleeding index,gingival index,and probing depth at 1,2,4,and 8 weeks after surgery,and the levels of serum TNF-α,IL-6 and IL-1β at 1,2,and 4 weeks after surgery of the rats in the Ag-HA coating group and HA coating group were significantly lower than those before surgery and in the periodontitis group at the same period(all P<0.05),the probing depth at 1,2,4 weeks after surgery,and the levels of serum TNF-α,IL-6 and IL-1β at 4 and 8 weeks after surgery of the rats in the Ag-HA coating group were significantly lower than those in the HA group at the same period;the levels of serum ALP at 1,2,4 and 8 weeks after surgery of the rats in the Ag-HA coating group and HA coating group were significantly higher than those in the periodontitis group at the same period(all P<0.05),there was no significant difference in the level of serum ALP at 1,2 weeks after surgery between Ag-HA coating group and HA coating group(all P>0.05),the levels of serum ALP at 4 and 8 weeks after surgery and the implant-bone incorporation rate at 2,4 and 8 weeks after surgery in the Ag-HA coating group were significantly higher than those in the HA coating group at the same period(all P<0.05).Conclusion Ag-HA-coated implants could alleviate the inflammation around teeth and improved implant-osteobonding ability in rats with periodontitis.
