新辅助化疗联合胃癌根治术治疗进展期胃癌的临床疗效

Clinical efficacy of neoadjuvant chemotherapy combined with radical gastrectomy in the treatment of advanced gastric cancer

目的探究新辅助化疗联合胃癌根治术治疗进展期胃癌的可行性及有效性。方法回顾性分析2017年2月至2020年2月于本院就诊的70例进展期胃癌患者的临床资料,根据治疗方法不同分为观察组与对照组,每组35例。对照组接受腹腔镜胃癌根治术治疗,观察组在对照组基础上接受新辅助化疗,比较两组手术疗效、手术切除情况、不良反应发生情况及生命质量。结果观察组治疗有效率与疾病控制率分别为88.57%、97.14%,均明显高于对照组的62.86%、77.14%,差异有统计学意义(P<0.05)。观察组手术切除总完成率为100.00%,明显高于对照组的82.86%,差异有统计学意义(P<0.05)。两组不良反应发生率比较差异无统计学意义。术后6个月,观察组健康状况调查简表(SF-36)评分明显高于对照组,差异有统计学意义(P<0.05)。结论新辅助化疗联合胃癌根治术治疗进展期胃癌患者效果确切,可有效提升疾病控制率、肿瘤切除率及患者生命质量,安全性较高。

Objective To explore the feasibility and effectiveness of neoadjuvant chemotherapy combined with radical gastrectomy in the treat-ment of advanced gastric cancer.Methods The clinical data of 70 patients with advanced gastric cancer who treated in our hospital from February 2017 to February 2020 were retrospectively analyzed,they were divided into the observation group and the control group according to different treat-ment methods,with 35 cases in each group.The control group received laparoscopic radical gastrectomy,while the observation group received neoad-juvant chemotherapy on the basis of the control group,the surgical efficacy,surgical resection status,occurrence of adverse reactions,and quality of life were compared between the two groups.Results The treatment effective rate and disease control rate in the observation group were 88.57%and 97.14%,respectively,which were significantly higher than 62.86%and 77.14%in the control group,and the differences were statistically significant(P<0.05).The total completion rate of surgical resection in the observation group was 100.00%,which was significantly higher than 82.86%in the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in the incidence of adverse re-actions between the two groups.At 6 months after surgery,the the MOS item short from health survey(SF-36)score in the observation group was sig-nificantly higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Neoadjuvant chemotherapy combined with radical gastrectomy is effective in the treatment of advanced gastric cancer patients,and can effectively improve disease control rate,tumor resection rate,and quality of life of patients,and has high safety.
