快速康复外科护理在后腹腔镜下输尿管切开取石术中的应用效果

Application effect of fast track surgery nursing in retroperitoneal laparoscopic ureterolithotomy

目的探讨快速康复外科护理在后腹腔镜下行输尿管切开取石术中的临床价值。方法选取2019年1月至2020年12月清远市人民医院泌尿外科收治的64例输尿管结石患者作为研究对象,采取随机数字表法分为研究组与对照组,各32例。对照组采用常规护理,研究组采用快速康复外科护理,比较两组围手术期指标、并发症发生率及心理状态。结果两组手术时间比较差异无统计学意义;研究组首次下床时间、首次排气时间、首次排便时间、首次进食时间、住院时间均短于对照组,差异有统计学意义(P<0.05)。研究组术后并发症发生率为6.25%,低于对照组的31.25%,差异有统计学意义(P<0.05)。护理后,研究组焦虑自评量表(SAS)、抑郁自评量表(SDS)评分均低于对照组,差异有统计学意义(P<0.05)。结论快速康复外科护理应用于后腹腔镜下输尿管切开取石术中,可促进术后各项生理功能恢复,提升手术治疗安全性,改善患者不良心理,加快术后康复进程。

Objective To investigate the clinical value of fast track surgery(FTS)nursing in retroperitoneal laparoscopic ureterolithotomy.Methods 64 patients with ureteral calculi who were admitted to the department of Urology,Qingyuan People's Hospital from January 2019 to De-cember 2020 were selected as the research subjects,and they were divided into the study group and the control group according to the random num-ber table method,with 32 cases in each group.The control group was given routine nursing and the study group was given FTS nursing,the perioper-ative indexes,complication rates and psychological status were compared between the two groups.Results There was no significant difference in operation time between the two groups;the first time of getting out of bed,first time of exhaust,first time of defecation,first time of eating,and hos-pitalization time in the study group were shorter than those in the control group,and the differences were statistically significant(P<.05).The inci-dence of postoperative complication in the study group was 6.25%,which was significantly lower than 31.25%in the control group,and the differ-ence was statistically significant(P<0.05).After nursing,the self-rating anxiety scale(SAS)and self-rating depression scale(SDS)scores in the study group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion The ap-plication of FTS nursing in retrolaparoscopic ureterolithotomy can promote the recovery of various physiological functions after surgery,improve the safety of surgical treatment,reduce the adverse psychology of patients,and accelerate the postoperative rehabilitation process.
