摘要
目的探究盆底超声检查联合盆底肌力评估分析阴道分娩及剖腹产产妇对女性盆底功能的影响。方法选取2019年6月至2021年6月于本院行盆底肌力评估与盆底超声检查的526例产妇作为研究对象,依据产妇分娩方式不同分为阴道分娩组(n=368)及剖腹产组(n=158)。比较两组盆底超声检查指标(最大Valsalva动作膀胱颈角度、静息状态膀胱颈角度、膀胱颈移动度、膀胱旋转角度)及盆底肌力指标(持续收缩力、最大收缩力)。结果经盆底超声检查结果显示,剖腹产组最大Valsalva动作膀胱颈角度、静息状态膀胱颈角度、膀胱旋转角度小于阴道分娩组,膀胱颈移动度短于阴道分娩组(P<0.05)。经盘底肌力评估显示,剖腹产组持续收缩力及最大收缩力均大于阴道分娩组(P<0.05)。结论盆底肌力评估联合盆底超声检查分析发现不同分娩方式产妇盆底功能存在较大差异,剖腹产产妇盆底功能较好,说明剖腹产对产妇盆底功能的影响较小,形成的肌力损伤较小。
Objective To explore the effect of pelvic floor ultrasound combined with pelvic floor muscle strength assessment on the function of female pelvic floor during vaginal delivery and caesarean section.Methods 526 women who underwent pelvic floor muscle strength assessment and ultrasound examination in our hospital from June 2019 to June 2021 were selected as the study subjects,and were divided into the vaginal delivery group(n=368)and the cesarean section group(n=158)according to different ways of delivery.The pelvic floor ultrasound indexes(maximum Valsal-va motion bladder neck angle,resting bladder neck angle,bladder neck movement degree,bladder rotation angle)and pelvic floor muscle strength in-dexes(continuous contraction force,maximum contraction force)were compared between the two groups.Results The results of pelvic floor ultra-sound showed that the maximum Valsalva movement bladder neck angle,resting bladder neck angle and bladder rotation angle in the cesarean sec-tion group were lower than the vaginal delivery group,bladder neck movement was shorter than the vaginal delivery group(P<0.05).The continu-ous and maximum contractility in the cesarean section group were higher than the vaginal delivery group(P<0.05).Conclusion Pelvic floor mus-cle strength assessment combined with pelvic floor ultrasound analysis found that there were great differences in pelvic floor function among women with different delivery modes,and those with cesarean section had better pelvic floor function,indicating that cesarean section had less impact on pel-vic floor function and less muscle strength damage.
