脑卒中后排尿障碍患者应用电针骶腹八穴治疗对膀胱残余尿量及单次平均尿量的效果分析

Analysis of the effect of electro-acupuncture on the residual urine volume and the single average urine volume of the urinary bladder in patients with voiding disorders after strok

目的:分析脑卒中后排尿障碍患者应用电针“骶腹八穴”治疗对膀胱残余尿量及单次平均尿量的效果。方法:选取2020年1月—2022年12月宁夏回族自治区中医医院暨中医研究院针灸科收治的脑卒中后排尿障碍患者100例,随机分为试验组(n=50)和对照组(n=50)。对照组给予传统针刺治疗,试验组采用电针“骶腹八穴”(骶四穴、腹四穴)治疗。比较两组患者的排尿功能指标,尿潴留、尿失禁症状,尿动力学指标,生活质量(QOL)及神经功能缺损(CSS)评分。结果:治疗前,两组患者的各项指标均无显著差异(P>0.05)。治疗后,试验组患者的排尿频率、尿量、膀胱容量、排尿时间、最大尿道关闭压、功能性尿道长度以及QOL评分均高于对照组患者,尿失禁次数少于对照组,尿潴留、尿失禁等症状评分以及CSS评分均低于对照组(P<0.05)。结论:电针“骶腹八穴”治疗脑卒中后排尿障碍,可缓解患者尿潴留、尿失禁等症状,促进其排尿功能恢复,同时提高其生活质量,改善其神经功能。

Objective To analyze the effect of electroacupuncture on residual urine volume and urinary bladder in patients after stroke.Methods A total of 100 patients admitted to the TCM Hospital of Ningxia Hui Autonomous Region from January 2020 to December 2022 were selected as research subjects and randomly divided into test group(n=50 cases)and control group(n=50 cases).The control group was treated with traditional acupuncture,and the test group was given electroacupuncture“sacroabdominal eight points”(four sacral acupoints and four abdominal acupoints)treatment.The efficacy judgment of urinary retention,urinary incontinence,urodynamic evaluation,quality of life(QOL),and neurological deficit(CSS)were compared between the two groups.Results There were no significant difference in the two groups before treatment(P>0.05).After the treatment,the urination frequency,urinary volume,bladder capacity,urination time,maximum urethral closure pressure,functional urethra length and QOL score of the test group were higher than that of the control group,and the number of urinary incontinence was less than that of the control group,and the symptom score of urinary retention and CSS score were lower than that of the control group,which was statistically significant(P<0.05).Conclusions The use of electroacupuncture“sacro-abdominal eight points”for patients with voiding disorders after stroke can relieve urinary retention and urinary incontinence,promote the recovery of urinary function,improve the quality of life and improve the nerve function.
