中频止鼾器治疗OSAHS的临床效果观察

摘要 目的:通过CT影像证实中频止鼾器治疗阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的临床效果。方法:选取2023年3—6月于上海市金山区亭林医院就诊的40例OSAHS患者,均予以中频止鼾器治疗。治疗1周后,分别在其清醒、打鼾、中频止鼾器治疗时完成口咽部上气道CT检查,选择打鼾时患者软腭后区、舌后区气道最狭窄处作为定位测量点,分别测量计算不同状态下同一位置气道的横截面积并进行对比分析。结果:在清醒、打鼾、中频止鼾器治疗时,OSAHS患者软腭后气道最狭窄处的横截面积分别为(95.10±14.07)、(69.46±13.69)、(108.41±21.91)mm^(2),舌后区气道最狭窄处的横截面积分别为(223.77±23.46)、(166.91±19.47)、(244.11±22.05)mm^(2)。与清醒时比较,OSAHS患者打鼾时的软腭后气道、舌后区气道横截面积均减小(P<0.05);与打鼾时比较,OSAHS患者在止鼾器治疗时软腭后气道、舌后区气道横截面积均增加(P<0.05)。结论:OSAHS患者睡眠状态下软腭后、舌后区的气道横截面积缩小,使用中频止鼾器治疗时的气道横截面积增大。因此,中频止鼾器可以通过改善OSAHS患者口咽部上气道的狭窄程度发挥治疗作用。 Objective To confirm the clinical effect of medium frequency anti-snoring device in the treatment of obstructive sleep apnea-hypopnea syndrome(OSAHS)through CT scan.Methods A total of 40 patients with OSAHS in Tinglin Hospital of Jinshan District in Shanghai from March to June 2023 were selected as the study subjects,and all of them were treated with medium frequency anti-snoring device.One week after the treatment,the upper airway CT was performed in their waking state,snoring state,and the treatment state with the medium frequency anti-snoring device.The narrowest part of the airway in the posterior area of the soft palate and the posterior area of the tongue during snoring was selected as the measurement point.The cross-sectional area of the airway at the same position in different states was measured and calculated respectively,and comparative analysis was conducted.Results When the OSAHS patients were conscious,snoring and treated with medium frequency anti-snoring device,the crosssectional area of the narrowest part of the airway behind the soft palate was(95.10±14.07)mm^(2),(69.46±13.69)mm^(2) and(108.41±21.91)mm^(2),respectively.The cross-sectional areas of the narrowest part of the retrolingual airway was(223.77±23.46)mm^(2),(166.91±19.47)mm^(2) and(244.11±22.05)mm^(2),respectively.Being compared with the waking state,the cross-sectional area of the airway behind the soft palate and retrolingual airway decreased during snoring(P<0.05).Being compared with the status of snoring,the cross sectional areas of the airway behind the soft palate and retrolingual airway increased with the treatment of medium frequency anti-snoring device(P<0.05).Conclusions The cross sectional area of the retropalatal and retrolingual airway is reduced in the sleep state for OSAHS patients,and it is increased after the treatment with medium frequency anti-snoring device.Therefore,the medium frequency anti-snoring device plays a therapeutic role by improving the degree of oropharyngeal airway stenosis in OSAHS patients.
