恐动症对防旋股骨近端髓内钉术后影响因素及干预措施研究进展

Research progress on the influencing factors and intervention measures for the postoperative kinesiophobia after proximal femoral nail anti-rotation

本文对防旋股骨近端髓内钉术后患者恐动症的影响因素和干预措施进行综述,通过阐述髋部骨折和股骨粗隆间骨折的流行病学以及防旋股骨近端髓内钉术的优点,由此引出手术的创伤会导致患者出现疼痛,进而患者可能产生对受伤和手术后疼痛的恐惧,因对运动可能带来的疼痛而出现恐动症。总结归纳国内外防旋股骨近端髓内钉术后恐动症患者的相关研究进展,恐动症的概念、来源、负性情绪、相关影响因素、测量工具以及干预方法,目的是为了减轻患者在防旋股骨近端髓内钉术后产生的疼痛和恐惧感,增强患者锻炼的依从性和自主性,促进患者髋关节功能的早期恢复,同时也为医护人员预见和干预防旋股骨近端髓内钉术后患者出现恐动症提供参考依据,以期帮助患者降低预后不良的风险,提高患者生活质量。

This review describes the influencing factors and intervention measures for the postoperative kinesiophobia after proximal femoral nail anti-rotation(PFNA).By explaining the epidemiology of hip fractures and intertrochanteric fractures,and the merits of PFNA,the cause of postoperative kinesiophobia is introduced.Surgery-induced trauma causes pain,and patients may fear from injury and surgery-induced pain.As a result,kinesiophobia occurs due to the fear of exercise-induced pain.This review summarizes relevant studies on kinesiophobia following PFNA at home and abroad.We interpret the concept,source,negative emotion,influencing factors,measurement tools and intervention methods of kinesiophobia following PFNA,aiming to alleviate the fear of postoperative pain,enhance the compliance and autonomy of postoperative exercise,and promote the early recovery of hip function.Besides,this review provides reference for the prediction and intervention of kinesiophobia following PFNA by healthcare staff,aiming to reduce the risk of poor prognosis and enhance the quality of life.
