基于舌苔辨证的病证结合治疗急性缺血性脑卒中的临床疗效分析

Clinical effect analysis of combination of disease and syndrome treatment based on tongue coating differentiation with acute ischemic stroke

目的:评价基于舌苔行简化临床辨证的病证结合治疗急性缺血性脑卒中(AIS)的疗效。方法:纳入AIS患者102例并随机分为对照组(50例)和试验组(52例),两组均予以西医基础治疗,试验组予基于舌苔辨证的中药治疗,对照组予中药安慰剂治疗,治疗14 d,治疗后第(90±3)天进行随访。比较两组预后、并发症、复发率和病死率以及经济学指标。结果:治疗后两组患者的NIHSS评分显著降低(P<0.05),BI评分及mRS≤2患者百分比显著升高(P<0.05)。治疗期间,试验组呼吸道感染发生率显著低于对照组(P<0.05),试验组住院日均费用较对照组有降低趋势。结论:本研究中的病证结合治疗对AIS的预后未见优势,但能降低患者呼吸道感染发生率,并有减少日均住院费用的趋势,值得关注。

Objective:To evaluate the efficacy of combining disease and syndrome treatment for acute ischemic stroke(AIS)through simplified clinical syndrome differentiation based on tongue coating.Methods:A total of 102 AIS patients were randomly divided into two groups:control(50 cases)and test(52 cases).Both groups received basic western medicine treatment.In addition,the test group was administered traditional Chinese medicine(TCM)base on tongue coating differentiation,while the control group was given a placebo of TCM.The treatment lasted for 14 days and the follow-up period was(90+3)days.The two groups were compared in terms of prognosis,complications,recurrence rate,mortality rate,and economic indicators.Results:The NIHSS score decreased significantly(P<0.05),the Barthel index(BI)score and the proportion of mRS scores 2 and below were significant improved in both groups after treatment(P<0.05).Besides,the test group had a significantly lower incidence of respiratory tract infection during the treatment period compared to the control group(P<0.05).And the average daily hospitalization expenses in the experimental group showed a downward trend.Conclusion:While the combination of diseases and syndromes in this study may not provide any significant advantage in the prognosis of AIS,it does show potential in reducing the incidence of respiratory tract infections and lowering the average daily hospitalization cost.Further investigation into this area is warranted.
