原发性高血压患者外周血miR-126、miR-29b、miR-18b变化及与血管内皮损伤的关系 被引量:1

Changes of MiR-126,MiR-29b and MiR-18b in Peripheral Blood of Patients with Essential Hypertension and Their Relationship with Vascular Endothelial Injury

摘要 目的 探究原发性高血压患者外周血miRNA-126(miR-126)、miRNA-29b(miR-29b)、miRNA-18b(miR-18b)变化及与血管内皮损伤的关系。方法 采取病例对照研究,选择2021年9月至2022年9月收治的原发性高血压病例87例(高血压组)和同期体检的健康对照人群80名(对照组)。入选者均采集外周血标本,采用实时荧光定量聚合酶链反应(polymerase chain reaction, PCR)技术测定外周血miR-126、miR-29b、miR-18b表达水平,采用硝酸还原酶法测定血清一氧化氮(notric oxide, NO)水平,采用酶联免疫吸附法测定血清内皮素1(endothelin 1, ET-1)、血管性血友病因子(von Willebrand factor,vWF)水平。分析高血压组与对照组的检测结果差异及不同高血压分级患者间的检测结果差异,采用Spearman相关分析法分析原发性高血压外周血miR-126、miR-29b、miR-18b水平与NO、ET-1、vWF的关系。结果 高血压组患者高血压家族史比例、收缩压和舒张压水平均高于对照组,差异有统计学意义(P <0.05);高血压组外周血miR-126相对表达量、血清NO水平低于对照组,而外周血miR-29b、miR-18b相对表达量和血清ET-1、vWF水平高于对照组,组间比较差异有统计学意义(P <0.05)。随着高血压分级增加,收缩压、舒张压、总胆固醇、低密度脂蛋白胆固醇水平逐渐升高,两两比较差异有统计学意义(P <0.05);随着高血压分级增加,外周血miR-126相对表达量和血清NO水平逐渐降低,外周血miR-29b、miR-18b相对表达量和血清ET-1、vWF水平均逐渐升高,两两比较差异有统计学意义(P <0.05)。外周血miR-126相对表达量与血清NO水平呈正相关,与血清ET-1、vWF水平成负相关;miR-29b、miR-18b相对表达量均与NO水平呈负相关,与血清ET-1、vWF水平正相关(P <0.05)。结论 原发性高血压患者外周血miR-126相对表达量异常降低,而外周血miR-29b、miR-18b相对表达量均异常增加,且三者均与高血压分级、血管内皮损伤具有显著相关性。 Objective To explore the changes of peripheral blood miRNA-126(miR-126),miRNA-29b(miR-29b)and miRNA-18b(miR-18b)in patients with essential hypertension and their relationship with vascular endothelial injury.Methods Case-control study was conducted to select 87 patients with essential hypertension(hypertension group)and 80 healthy controls with physical examination(control group)from September 2021 to September 2022.Peripheral blood samples were collected from all cases.The expression levels of miR-126,miR-29b and miR-18b in peripheral blood were determined by real-time fluorescence quantitative polymerase chain reaction(PCR),the level of serum nitric oxide(NO)was detected by nitrate reductase method,and the levels of serum endothelin 1(ET-1)and von Willebrand factor(vWF)were determined by enzyme-linked immunosorbent assay.The differences in detection results between hypertension group and control group and the differences in detection results among patients with different hypertension grades were analyzed.Spearman correlation analysis was used to analyze the relationship between levels of miR-126,miR-29b and miR-18b in peripheral blood of essential hypertension and NO,ET-1 and vWF.Results The proportion of family history of hypertension,systolic blood pressure and diastolic blood pressure in hypertension group were higher than those in control group(P<0.05).The relative expression level of peripheral blood miR-126 and the serum NO level in hypertension group were lower while the relative expression levels of peripheral blood miR-29b and miR-18b and the levels of serum ET-1 and vWF were higher compared with those in control group(P<0.05).With the increase of hypertension grade,the systolic blood pressure,diastolic blood pressure,total cholesterol and low density lipoprotein cholesterol were increased gradually,and the differences in pairwise comparison were statistically significant(P<0.05).With the increase of hypertension grade,the relative expression level of peripheral blood miR-126 and the serum NO level were reduced gradually while the relative expression levels of miR-29b and miR-18b in peripheral blood and the levels of serum ET-1 and vWF were enhanced gradually,and the differences were statistically significant between any two grades(P<0.05).The relative expression level of miR-126 in peripheral blood was positively correlated with serum NO level and negatively correlated with serum ET-1 and vWF levels,and the relative expression levels of miR-29b and miR-18b were negatively correlated with NO level and were positively correlated with serum ET-1 and vWF levels(P<0.05).Conclusion The relative expression level of miR-126 in peripheral blood of patients with essential hypertension is decreased abnormally while the relative expression levels of miR-29b and miR-18b in peripheral blood are increased abnormally,and the above three indicators are significantly correlated with hypertension grading and vascular endothelial injury.
