渐进式康复护理路径在经皮椎间孔镜治疗腰椎间盘突出症中的应用效果

Application effect of progressive rehabilitation nursing pathway on lumbar disc herniation treated by percutaneous transforaminal endoscopy

目的探讨渐进式康复护理路径在经皮椎间孔镜治疗腰椎间盘突出症(LDH)中的应用效果。方法选择2019年2月至2022年2月收治的114例LDH患者为研究对象,根据随机抽样法将其分为对照组和研究组,各57例。两组均行经皮椎间孔镜治疗,对照组实施常规康复护理,研究组实施渐进式康复护理路径。比较两组的护理效果。结果研究组的康复总依从率高于对照组(P<0.05)。术后3个月,研究组的椎间隙高度、硬膜外压痕纵轴值、两侧腰大肌横截面面积差值及两侧多裂肌横截面面积差值低于对照组(P<0.05)。术后3个月,研究组的日本骨科协会(JOA)、Roland-Morris功能障碍调查表(RMQ)评分及腰椎关节活动度(ROM)高于对照组,视觉模拟评分法(VAS)评分低于对照组(P<0.05)。研究组的康复优良率高于对照组(P<0.05)。结论渐进式康复护理路径更有利于提高LDH患者的康复依从性,促进腰椎功能恢复,在经皮椎间孔镜治疗LDH中具有较高的应用价值。

Objective To explore the application effect of progressive rehabilitation nursing pathway on lumbar disc herniation(LDH)treated by percutaneous transforaminal endoscopy.Methods A total of 114 patients with LDH admitted from February 2019 to February 2022 were selected as the research objects.According to the random sampling method,the patients were divided into control group and study group,with 57 cases in each group.Both groups were treated with percutaneous transforaminal endoscopy.The control group was given routine rehabilitation nursing,and the study group was given progressive rehabilitation nursing pathway.The nursing effects of the two groups were compared.Results The total compliance rate of rehabilitation in the study group was higher than that in the control group(P<0.05).At 3 months after operation,the height of intervertebral space,the longitudinal axis value of epidural indentation,the difference of cross-sectional area of bilateral psoas major and the difference of cross-sectional area of bilateral multifidus in the study group were lower than those in the control group(P<0.05).At 3 months after operation,the scores of Japanese Orthopaedic Association(JOA),Roland-Morris Disability Questionnaire(RMQ)and lumbar range of motion(ROM)in the study group were higher than those in the control group,and the Visual Analogue Scale(VAS)score was lower than that in the control group(P<0.05).The excellent and good rate of rehabilitation in the study group was higher than that in the control group(P<0.05).Conclusion The progressive rehabilitation nursing pathway is more conducive to improving the rehabilitation compliance of LDH patients and promoting the recovery of lumbar function,which has high application value in LDH treated by percutaneous transforaminal endoscopy.
