耳穴疗法治疗原发性痛经的Meta分析

Meta-analysis of the efficacy of ear acupoint therapy on primary dysmenorrhea

目的 系统评价耳穴疗法(包括耳穴压豆、耳针、耳穴揿针等疗法)治疗原发性痛经的疗效。方法 计算机检索The Cochrane Library、PubMed、Embase、CBM、维普、万方和CNKI数据库,搜集有关耳穴疗法治疗原发性痛经的随机对照试验(RCT),检索时限从2013年1月至2023年2月28日。由2名研究人员独立进行文献筛选、资料提取和纳入研究的偏倚风险评价,而后采用Review Manager 5.0及StataSE 12.0软件进行Meta分析。结果 最终纳入15篇文献,共计16个研究,包括1232例患者,接受耳穴治疗的原发性痛经患者的总有效率[RR=1.21,95%CI(1.12,1.30),P<0.01]、痛经症状量表(CMSS)[MD=-1.76,95%CI(-2.79,-0.73),P<0.01]、视觉模拟评分法(VAS)[MD=-0.97,95%CI(-1.73,-0.20),P<0.01],差异均有统计学意义。结论 耳穴疗法与常规治疗相比,可以提高原发性痛经的疗效,能够改善原发性痛经的临床症状。

Objective To systematically review the efficacy of ear acupoint therapy(including auricular point sticking.auriculotherapy、auricular intradermal needling,etc.)on primary dysmenorrhea.Methods We searched databases including The Cochrane Library,PubMed,Embase,CBM,VIP,Wan Fang Data and CNKI databases committed to randomized controlled trials(RCTS)of auricular acupoint therapy for primary dysmenorrhea,the search was conducted from January 2013 to February 28th 2023.Two reviewers independently screened literature,extracted data and assessed the risk of bias of included studies.Meta-analysis was performed by using StatasE12 software.Results A total of 15 studies were included,involving a total of 16 studies and 1232 patients.The results of meta-analysis showed that the response rate of primary dysmenorrhea patients receiving auricular acupoint treatment[RR=1.21,95%CI(1.12,1.30),P<0.01]、the Cox menstrual symptom scale(CMSS)score[MD=-1.76,95%CI(-2.79,-0.73),P<0.01],the visual analog scale(VAS)score[MD=-0.97,95%CI(-1.73,-0.20),P<0.01].Sensitivity analysis showed that the results were stable and reliable.Conclusion Compared with conventional treatment,ear acupoint therapy can improve the overall effective rate of primary dysmenorrhea and improve its clinical symptoms.
