枸橼酸咖啡因与氨茶碱治疗早产儿呼吸暂停的临床效果 被引量:1

摘要 目的:探讨枸橼酸咖啡因与氨茶碱治疗早产儿呼吸暂停(apnea of prematurit,AOP)的临床效果。方法:选取2020年1月—2021年10月清流县总医院收治的100例AOP作为观察对象。根据随机数表法将其分为对照组和治疗组,各50例。两组给予一般治疗。对照组给予氨茶碱治疗,治疗组给予枸橼酸咖啡因治疗。比较两组相关指标、肺功能、神经发育功能及临床疗效。结果:治疗1周后,治疗组呼吸暂停次数少于对照组,正压通气时间及吸氧时间均短于治疗组,差异有统计学意义(P<0.05)。治疗组每分通气量、潮气量及潮气呼吸呼气峰流速(tidal breathing peak expiratory flow rate,PEF)、25%潮气量呼气流速(25%tidal volume expiratory flow rate,TEF25)、50%潮气量呼气流速(50%tidal volume expiratory flow rate,TEF50)、75%潮气量呼气流速(75%tidal volume expiratory flow rate,TEF75)均高于对照组,差异有统计学意义(P<0.05)。治疗组6个月、12个月智力发育指数(mental development index,MDI)评分和精神运动发育指数(psychomotor development index,PDI)评分均高于对照组,差异有统计学意义(P<0.05)。治疗组总有效率高于对照组,差异有统计学意义(P<0.05)。结论:与氨茶碱比,枸橼酸咖啡因治疗AOP的效果显著,能够改善肺功能和神经发育,缩短正压通气时间,是一种有效的治疗方案。 Objective:To explore the clinical effect of Caffeine Citrate and Aminophylline in the treatment of apnea of prematurity(AOP).Method:A total of 100 cases of AOP treated in Qingliu County General Hospital from January 2020 to October 2021 were selected as the observation objects.They were divided into control group and treatment group according to random number table method,with 50 cases in each group.Two groups were given general treatment.The control group was given Aminophylline treatment,and the treatment group was given Caffeine Citrate treatment.The relevant indexes,lung function,neurodevelopmental function and clinical efficacy of the two groups were compared.Result:After 1 week of treatment,the number of apnea in the treatment group was less than that in the control group,and the time of positive pressure ventilation and oxygen inhalation were shorter than those in the treatment group,the differences were statistically significant(P<0.05).The minute ventilation volume and tidal volume,tidal breathing peak expiratory flow rate(PEF),25%tidal volume expiratory flow rate(TEF25),50%tidal volume expiratory flow rate(TEF50)and 75%tidal volume expiratory flow rate(TEF75)in treatment group were higher than those in control group,and the differences were statistically significant(P<0.05).The scores of mental development index(MDI)and psychomotor development index(PDI)at 6 months and 12 months in the treatment group were higher than those in the control group,and the differences were statistically significant(P<0.05).The total effective rate of the treatment group was higher than that of the control group,and the difference was statistically significant(P<0.05).Conclusion:Compared with Aminophylline,Caffeine Citrate has a significant effect in the treatment of AOP,which can improve lung function and nerve development,shorten positive pressure ventilation time,and is an effective treatment plan.
