体检老年糖尿病人群NPDR危险因素及管理对策探讨

Exploration of Risk Factors and Management Countermeasures of NPDR in Elderly Diabetic Population Undergoing Physical Examination

目的:探讨体检老年糖尿病(DM)人群非增生性糖尿病视网膜病变(NPDR)危险因素及管理对策。方法:对2020年11月—2021年12月在湖北省中西医结合医院健康管理中心进行体检的854例老年DM患者的一般资料进行回顾性分析,将其分为NPDR组(n=168)和非NPDR组(n=686)。收集两组一般资料和实验室相关指标,通过多因素logistic回归分析老年DM患者发生NPDR的危险因素。结果:两组文化程度、DM病程、空腹血糖(FPG)、餐后2 h血糖(2 h PG)、血肌酐(Scr)、血尿氮素(BUN)、糖化血红蛋白(HbA1c)、血尿酸水平比较,差异有统计学意义(P<0.05)。logistic回归模型多因素分析结果显示,文化程度、DM病程、FPG、HbA1c水平均是老年DM患者发生NPDR的独立影响因素(P<0.05)。结论:体检老年DM患者发生NPDR的危险因素包括文化程度、DM病程、FPG、HbA1c水平。在临床上应对相关危险因素进行有效控制,采取有效措施以降低NPDR发生风险。

Objective:To investigate the risk factors and management countermeasures of non-proliferative diabetic retinopathy(NPDR)in elderly diabetic(DM)population undergoing physical examination.Method:The general data of 854 elderly DM patients who underwent physical examination in the Health Management Center of Hubei Hospital of Integrated Traditional and Western Medicine from November 2020 to December 2021 were retrospectively analyzed and divided into NPDR group(n=168)and non-NPDR group(n=686).The general data and laboratory related indicators of two groups were collected,and the risk factors of NPDR in elderly DM patients were analyzed by multivariate logistic regression.Result:There were statistically significant differences between two groups in the education level,duration of DM disease,fasting plasma glucose(FPG),2 h postprandial blood glucose(2 h PG),blood creatinine(Scr),blood urea nitrogen(BUN),glycated haemoglobin(HbA1c)and blood uric acid levels(P<0.05).Multi-factor logistic regression analysis results showed that education level,duration of DM disease,FPG and HbA1c levels were independent influencing factors for the occurrence of NPDR in elderly DM patients(P<0.05).Conclusion:Risk factors for the development of NPDR in elderly patients undergoing physical examination include education level,duration of DM disease,FPG,and HbA1c level.The risk factors should be effectively controlled in clinical practice and effective measures should be taken to reduce the risk of NPDR.
