基于引气归元理论温针灸治疗围绝经期心脾两虚型失眠的临床研究

Clinical study on the treatment of peri-menopausal insomnia caused by deficiency of both heart and spleen with warm acupuncture based on the theory of inducing Qi to return to the source

目的:分析基于引气归元理论进行温针灸治疗围绝经期心脾两虚型失眠的临床效果。方法:选择2022年1月—2023年6月就诊于广东医科大学顺德妇女儿童医院更年期专科门诊的80例心脾两虚型围绝经期失眠(PMI)患者作为研究对象,按随机数字表法分为两组,各40例。对照组给予药物治疗(口服归脾丸),治疗组在对照组治疗基础上联合引气归元穴温针灸治疗。比较两组患者治疗前后匹兹堡睡眠质量指数(PSQI)、更年期症状量表(改良Kupperman量表)评分及临床疗效。结果:治疗后,治疗组总有效率(95.00%)高于对照组(77.50%),差异有统计学意义(P<0.05)。治疗后,两组患者PSQI总分及各维度评分低于治疗前,治疗组PSQI总分及各维度评分低于对照组,差异有统计学意义(P<0.05);两组患者改良Kupperman量表评分低于治疗前,治疗组改良Kupperman量表评分低于对照组[(11.36±2.14)分vs(.14.93±2.32)分],差异有统计学意义(P<0.05)。结论:基于引气归元理论进行温针灸能够有效缓解围绝经期心脾两虚型失眠症患者症状,改善睡眠质量,提高临床疗效。

Objective:To analyze the clinical effects of warm acupuncturein treating peri-menopausal insomnia(PMI)caused by deficiency of both heart and spleen based on the theory of inducing Qi to return to the source.Methods:80 PMI patients with deficiency of both heart and spleen in the menopause specialist clinic of Shunde Women′s and Children′s Hospital of Guangdong Medical University from January 2022 to June 2023 were selected as the research objects and divided into two groups according to random number table method,with 40 cases in each group.The patients in the control group were treated with medication(oral administration of Guipi Pill),and the patients in the treatment group were treated with warm acupuncture at the acupoints for inducing Qi to return to the source on the basis of the treatment of the control group.The Pittsburgh Sleep Quality Index(PSQI)score,menopausal symptom scale(modified Kupperman)score and clinical efficacy were compared between the two groups before and after treatment.Results:After treatment,the total effective rate of the treatment group was 95.00%,which was higher than 77.50%of the control group,and the difference was statistically significant(P<0.05)After treatment,the total PSQI score and each dimension score of the two groups were lower than those before treatment,and the total PSQI score and each dimension score of the treatment group were lower than those of the control group,with the statistically significant differences(P<0.05).The modified Kupperman scale scores of the two groups were lower than that before treatment,and the modified Kupperman scale score of the treatment group was lower than that of the control group[(11.36±2.14)vs.(14.93±2.32)],with the statistically significant differences(P<0.05).Conclusion:Warm acupuncture based on the theory of inducing Qi to return to the source can effectively relieve the symptoms of PMI patients with deficiency of both heart and spleen,improve sleep quality and clinical efficacy.
