不同剂型异烟肼与利福平联合治疗肺结核的临床效果分析

Clinical Effect of Different Dosage Forms of Isoniazid Combined with Rifampicin in the Treatment of Pulmonary Tuberculosis

目的:探究不同剂型异烟肼与利福平联合治疗肺结核的临床效果。方法:选取2020年1月—2021年12月松桃县人民医院收治的肺结核患者78例为研究对象,按照均等单盲法分为对照组和研究组,各39例。对照组给予利福平胶囊联合异烟肼片治疗,研究组给予利福平注射液联合异烟肼注射液治疗。比较两组患者临床指标、血清细胞因子、免疫功能指标、不良反应发生情况。结果:研究组病灶吸收率高于对照组,差异有统计学意义(P<0.05);治疗后,两组患者白细胞介素-10、白细胞介素-18、肿瘤坏死因子-α、γ干扰素水平均降低,且研究组低于对照组,差异有统计学意义(P<0.05);治疗后,两组患者CD4^(+)、CD4^(+)/CD8^(+)水平均升高,CD8^(+)水平降低,且研究组CD4^(+)、CD4^(+)/CD8^(+)水平高于对照组,CD8^(+)水平低于对照组,差异有统计学意义(P<0.05);两组患者不良反应发生率比较,差异无统计学意义(P>0.05)。结论:对肺结核患者采用利福平注射液联合异烟肼注射液治疗效果较利福平胶囊联合异烟肼片显著,利于病灶吸收率提高,可改善机体血清细胞因子和免疫功能,但从操作方便、用药配合度等方面分析,可选用利福平胶囊联合异烟肼片治疗。

Objective:To explore the clinical effect of different dosage forms of isoniazid combined with rifampicin in the treatment of pulmonary tuberculosis.Methods:Seventy-eight patients with tuberculosis admitted to Songtao County People's Hospital from January 2020 to December 2021 were selected as the study objects,and were divided into control group and study group according to equal single blind method,thirty-nine cases each.The control group was treated with rifampicin capsule combined with isoniazid tablets,and the study group was treated with rifampicin injection combined with isoniazid injection.The clinical indexes,serum cytokines,immune function indexes and adverse reactions were compared between the two groups.Results:The lesion absorptivity of the study group was higher than that of the control group,and the difference was statistically significant(P<0.05);After treatment,the levels of interleukin-10,interleukin-18,tumor necrosis factor-α and gamma interferon in 2 groups were decreased,and the study group was lower than the control group,the difference was statistically significant(P<0.05);After treatment,the levels of CD4^(+)and CD4^(+)/CD8^(+)increased in both groups,while the levels of CD8^(+)decreased,and the levels of CD4^(+)and CD4^(+)/CD8^(+)in the study group were higher than those in the control group,while the levels of CD8^(+)were lower than those in the control group,with statistical significance(P<0.05);There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:The therapeutic effect of rifampicin injection combined with isoniazid injection is more significant than that of rifampicin capsule combined with isoniazid tablet in patients with pulmonary tuberculosis,which is conducive to improving lesion absorption rate and improving serum cytokines and immune function of the body.However,rifampicin capsule combined with isoniazid tablet can be selected for treatment from the aspects of convenient operation and drug compatibility.
