黄连温胆汤合星蒌承气汤联合神经介入取栓术治疗急性脑梗死临床观察

Huanglian Wendan Decoction and Xinglou Chengqi Decoction Combined with Neuro-interventional Thrombotomy in the Treatment of Acute Cerebral Infarction

目的观察黄连温胆汤合星蒌承气汤联合神经介入取栓术治疗急性脑梗死的临床疗效。方法将82例急性脑梗死患者随机分为2组,各41例,2组均采用神经介入取栓术治疗,对照组在术后采用抗凝等内科常规治疗,治疗组在对照组的基础上采用黄连温胆汤合星蒌承气汤治疗。结果2组治疗后症状评分、NIHSS评分、血液黏稠度均低于治疗前(P<0.05),且治疗组低于对照组(P<0.05)。2组治疗后Balthel指数高于治疗前(P<0.05),且治疗组高于对照组(P<0.05)。治疗组总有效率高于对照组(P<0.05)。结论黄连温胆汤合星蒌承气汤联合神经介入取栓术治疗急性脑梗死,能够改善神昏、偏瘫、失语程度及血液黏稠度,减轻神经功能缺损,临床疗效及提高生活质量。

Objective To observe the clinical effect of Huanglian Wendan decoction and Xinglou Chengqi decoction combined with neurointerventional on acute cerebral infarction.Methods 82 patients with acute cerebral infarction were randomly divided into two groups,with 41 patients in each group.Both groups were treated with digital subtraction angiography(DSA)guided interventional embolectomy.The control group was treated with anticoagulation and other conventional medical treatments after DSA guided interventional embolectomy.The treatment group was treated with Huanglian Wendan decoction and Xinglou Chengqi decoction on the basis of the control group.Both groups were treated for 7 days.Results After treatment,the scores of stupor,hemiplegia,aphasia,NIHSS,whole blood low shear viscosity,whole blood high shear viscosity and fibrinogen in the two groups were lower than those before treatment(P<0.05),and those in the treatment group were lower than those in the control group(P<0.05).After treatment,the Balthel index in both groups was higher than that before treatment(P<0.05),and that in the treatment group was higher than that in the control group(P<0.05).The total effective rate of the treatment group was higher than that of the control group(P<0.05).Conclusion Huanglian Wendan decoction and Xinglou Chengqi decoction combined with neuro-interventional thrombotomy can improve the degree of stupor,hemiplegia,aphasia and blood viscosity,reduce neural function defects,improve the quality of life and clinical effect in the treatment of acute cerebral infarction.
