麻防犀角地黄汤加减联合西药治疗寻常型银屑病疗效及对皮损面积和血清炎症因子水平的影响

Efficacy of modified Mafang Xijiao Dihuang Decoction Combined with Western Medicine in the treatment of pa-tients with psoriasis vulgaris and its effects on skin lesion area and serum inflammatory factors levels

目的:探讨麻防犀角地黄汤加减联合西药治疗寻常型银屑病患者的疗效及对皮损面积和血清炎症因子水平的影响。方法:将2020年7月~2021年7月我院收治的90例寻常型银屑病患者按照随机信封法分为2组,对照组(n=45例,维A酸乳膏),观察组(n=45例,麻防犀角地黄汤+维A酸乳膏),治疗时间为3个月,比较2组患者治疗总有效率、皮损面积、中医症状积分、血清炎症因子水平、患病后生活质量。结果:治疗3个月后观察组治疗总有效率大于对照组(P<0.05);观察组皮损面积、中医症状积分小于对照组(P<0.05);观察组白介素-2(IL-2)、白介素-6(IL-6)、白介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)水平小于对照组(P<0.05);观察组皮肤病生活质量指数表(DLQD)评分低于对照组(P<0.05)。结论:麻防犀角地黄汤+维A酸乳膏用于治疗寻常型银屑病可以提高治疗效果,减少皮损面积,减轻中医症状与炎症反应,提高患者的生活质量,可以在临床推广使用。

Objective:To investigate the efficacy of modified Mafang Xijiao Dihuang Decoction combined with western medi-cine on patients with psoriasis vulgaris and its effects on the area of skin lesions and the levels of serum inflammatory factors.Methods:A total of 90patients with psoriasis vulgaris who were treated in the hospital from July 2020to July 2021were divided into two groups according to the random envelope method,including the control group(n=45,tretinoin cream)and the observation group(n=45,Mafang Xijiao Dihuang decoction+tretinoin cream),with the treatment time of 3months.The total effective rate of treatment,skin lesion area,TCM symptoms scores,serum inflammatory factors and quality of life after illness were compared be-tween the two groups.Results:After 3months of treatment,the total effective rate of treatment in the observation group was higher than that of the control group(P<0.05).The skin area and scores of TCM symptoms of the observation group after 3months of treatment were lower than those of the control group(P<0.05).The levels of interleukin-2(IL-2),interleukin-6(IL-6),interleukin-8(IL-8)and tumor necrosis factor-α(TNF-α)were lower in the observation group than those in the control group after 3months of treatment(P<0.05).The scores of aspects of Dermatology Life Quality Index(DLQI)after 3months of treatment in the observation group were lower compared to the control group(P<0.05).Conclusion:Mafang Xijiao Dihuang decoction+tretinoin cream in treating psoriasis vulgaris can improve the therapeutic effect,reduce the area of skin lesions,relieve the TCM symptoms and inflammatory response and enhance the quality of life of patients,thus it can be widely used in clinical practice.
