AST、cTnⅠ和胆碱酯酶水平检测对急性有机磷农药中毒患者的临床意义

Clinical Significance of Detection of AST,cTnⅠ and Cholinesterase Levels in Patients with Acute Organophosphorus Pesticide Poisoning

目的 探讨谷草转氨酶(AST)、心肌肌钙蛋白Ⅰ(cTnⅠ)以及胆碱酯酶水平检测对急性有机磷农药中毒患者的临床意义。方法 选择70例急性有机磷农药中毒患者为观察组,同期健康体检人员70例为对照组,比较两组的AST、 cTnⅠ以及胆碱酯酶水平。观察组患者参考临床症状严重程度分为轻中度中毒亚组与重度中毒亚组,比较两亚组的AST、 cTnⅠ以及胆碱酯酶水平,分析AST、 c TnⅠ以及胆碱酯酶水平与患者病情严重程度的相关性。结果 观察组的AST、 cTnⅠ水平高于对照组,胆碱酯酶水平低于对照组(P<0.05)。重度中毒亚组的AST、 cTnⅠ水平高于轻中度中毒亚组,胆碱酯酶水平低于轻中度中毒亚组(P<0.05)。急性有机磷农药中毒患者的病情严重程度与AST、 cTnⅠ水平呈正相关,与胆碱酯酶水平呈负相关(P<0.05)。结论 急性有机磷农药中毒患者的AST、 cTnⅠ以及胆碱酯酶水平均发生异常改变,检测上述指标水平可鉴别疾病发生情况与病情严重程度。

Objective To explore the clinical significance of detection of aspartate aminotransferase(AST),cardiac troponinⅠ(cTnⅠ)and cholinesterase levels in patients with acute organophosphorus pesticide poisoning.Methods 70 patients with acute organophosphorus pesticide poisoning were selected as the observation group,70 healthy people were selected as the control group,and the levels of AST,cTnⅠand cholinesterase were compared between the two groups.Patients in the observation group were divided into mild to moderate poisoning subgroup and severe poisoning subgroup according to the severity of clinical symptoms,the levels of AST,cTnⅠand cholinesterase in the two subgroups were compared,and the correlations of the levels of AST,cTnⅠand cholinesterase with the severity of disease was analyzed.Results The AST and cTnⅠlevels in the observation group were higher than those in the control group,and cholinesterase level was lower than that in the control group(P<0.05).The AST and cTnⅠlevels in the severe poisoning subgroup were higher than those in the mild-to-moderate poisoning subgroup,and the cholinesterase level was lower than that in the mild-to-moderate poisoning subgroup(P<0.05).The disease severity of patients with acute organophosphorus pesticide poisoning was positively correlated with AST and cTnⅠlevels,and negatively correlated with cholinesterase level(P<0.05).Conclusions Patients with acute organophosphorus pesticide poisoning have abnormal change in the levels of AST,cTnⅠand cholinesterase.The detection of the above indicators can identify the occurrence and severity of the disease.
