血清PCT、腹腔AMY联合血清ALB早期预测十二指肠切除术后并发胰瘘的价值

Value of Serum PCT,Abdominal AMY Combined with Serum ALB in Early Prediction of Pancreatic Fistula After Duodenal Resection

目的 探讨血清降钙素原(PCT)、腹腔淀粉酶(AMY)联合血清白蛋白(ALB)早期预测十二指肠切除术后并发胰瘘的价值。方法 100例行十二指肠切除术的患者根据术后胰瘘发生情况分为胰瘘组和非胰瘘组,比较两组的血清PCT、腹腔AMY、血清ALB水平。术后胰瘘患者根据病情严重程度分为微瘘亚组和非微瘘亚组,比较两亚组的血清PCT水平。结果 100例患者术后胰瘘发生率为37.00%。术后3、 5、 7 d,胰瘘组的血清PCT、腹腔AMY、血清ALB水平均高于非胰瘘组(P<0.05)。术后1、 3、 5、 7 d,微瘘亚组的血清PCT水平低于非微瘘亚组(P<0.05)。结论 血清PCT、腹腔AMY检测能早期发现胰瘘,血清PCT检测能预测腹腔感染的严重程度,血清ALB水平可指导临床给予患者营养支持治疗。

Objective To explore the value of serum procalcitonin(PCT),abdominal amylase(AMY) combined with albumin(ALB) in early prediction of pancreatic fistula after duodenal resection.Methods According to the occurrence of postoperative pancreatic fistula,100 patients with duodenal resection were divided into pancreatic fistula group and non-pancreatic fistula group,and the levels of serum PCT,abdominal AMY and serum ALB were compared between the two groups.Patients with postoperative pancreatic fistula were divided into micro-fistula subgroup and non-micro-fistula subgroup according to the severity of the disease,and the serum PCT levels of the two subgroups were compared.Results The incidence of postoperative pancreatic fistula in 100 patients was 37.00%.At 3,5 and 7 days after surgery,the levels of serum PCT,abdominal AMY and serum ALB in pancreatic fistula group were higher than those in non-pancreatic fistula group(P<0.05).At 1,3,5 and 7 days after surgery,the serum PCT levels in the micro-fistula subgroup were lower than those in the non-micro-fistula subgroup(P<0.05).Conclusions Serum PCT and abdominal AMY detection can detect pancreatic fistula early,serum PCT detection can predict the severity of abdominal infection,and serum ALB level can guide clinical nutritional support treatment for patients.
